The Hop-a-Jet flight crew taking off without a takeoff clearance which resulted in a conflict with a JetBlue flight that had been cleared to land on an intersecting runway.

The flight crew's decision to continue the approach into known area of potentially severe weather (Thunderstorm), which resulted in the flight encountering a 30 knot cross wind, heavy rain, low-level wind shear, and hydroplaning on a ungrooved contaminated runway.

The failure of the flight crew to main directional control of the airplane after landing, resulting in the airplane going off the side of the runway and colliding with a taxiway sign, collapsing the nose landing gear, and causing substantial damage to the airplane. A factor in the accident was flight crew fatigue due to being on duty for about 17 hours 45 minutes.
