ON CLIMB OUT FROM DENVER, CENTENNIAL, (KAPA), THE AIRPLANE WOULD NOT PRESSURIZE. THE CREW ELECTED TO RETURN TO THE POINT OF DEPARTURE. MAINTENANCE WAS SUMMONED. IT WAS DETERMINED THAT THE PRESSURIZATION CONTROLLER HAD FAILED. AN OVERHAULED PRESSURIZATION CONTROLLER WAS INSTALLED AND WAS OPERATIONALLY TESTED ON THE GROUND. AN EVALUATION FLIGHT WAS DONE, AND THE PRESSURIZATION SYSTEM WAS FOUND TO OPERATE SATISFACTORY. AIRCRAFT WAS RETURNED TO SERVICE.
ENGINE #2 FUEL LIGHT ILLUMINATED AFTER 25 MINUTES OF FLIGHT AT FL320. FOLLOWED CHECKIST PROCEDURE. REDUCED POWER TO IDLE AND LIGHT WENT OUT. DIVERTED TO DEPARTURE AIRPORT, SCOTTSDALE, AZ (KSDL). CALLED MAINTENANCE FOR SUPPORT. REPLACED #2 ENGINE FUEL HIGH TEMP. SWITCH AND SWAPPED #1 DIFFERENTIAL PRESSURE SWITCH WITH #2 DIFFERENTIAL SWITCH FOR TROUBLESHOOTING. OPERATIONAL AND LEAK CHECK SATISFACTORY.
ON LANDING IN ALBANY NY (KALB), THE NOSE GEAR WOULD NOT EXTEND NORMALLY. EMERGENCY EXTENSION PROCEDURES WERE USED SUCCESSFULLY AND A NORMAL LANDING WAS MADE. THE AIRPLANE WAS FLOWN GEAR DOWN TO OXFORD CT (KOXC) WHERE FURTHER TROUBLESHOOTING AND REPAIR COULD BE DONE. (NOTE: THIS IS AN INTERMITTENT PROBLEM). THE NOSE GEAR SELECTOR VALVE WAS REPLACED AS A PRECAUTION. THE LANDING GEAR WAS CYCLED THROUGH 10 CYCLES AND 6 EMERGENCY EXTENSION CYCLES WITH NO DEFECTS. THE AIRPLANE WAS RETURNED TO SERVICE. ON 4/19/23 THE AIRPLANE WAS ENROUTE TO POMPANO BEACH, FL (KPMP) AND IT DIVERTED, FOR AN UNRELATED REASON, TO SUFFOLK, NY (KFOK), WHERE THE NOSE GEAR DID NOT NORMALLY EXTEND. EMERGENCY EXTENSION PROCEDURES WERE USED AND AN UNEVENTFUL LANDING WAS MADE, SUBSEQUENT EXTENSIVE TROUBLESHOOTING WAS ACCOMPLISHED AND AN INTERMITTENT LANDING GEAR CONTROL UNIT WAS REPLACED. TO DATE, OPERATION OF THE LANDING GEAR IS NORMAL.
AIRPLANE DEPARTED OPA LOCKA, FL (KOPF) UNEVENTFUL FLIGHT UNTIL LANDING AT MILWAUKEE, WI (KMKE). UPON LANDING AT KMKE, BOTH LEFT MAIN TIRES BLEW OUT. AS THE CREW TURNED THE AIRCRAFT ONTO THE TAXXIWAY, THEY NOTICED "MUSHY" HANDLING. THE AIRPLANE WAS STOPPED AND SUBSEQUENTLY REMOVED FROM THE TAXXIWAY WHEN REPLACEMENT WHEELS WERE OBTAINED. PATCHY ICE AND SNOW WERE REPORTED ON RUNWAYS AND TAXXIWAYS. CAUSE HAS YET TO BE DETERMINED. TWO TIRES,P/N 178K43-1 #1 S/N 20820762, #2 S/N 20960763
DEPARTING KCNM FLYING TO KLAS, CAPTAIN NOTICED BOTH ENGINE BLEED LIGHTS ILLUMINATED. THERE WERE NO INDICATIONS OF AN OVERHEAT CONDITION. ALL GAUGES, TEMPS AND INDICATORS WERE ALL NORMAL. CAPTAIN NOTIFIED ATI MAINTENANCE INFORMING THEM OF THE SITUATION, MAINTENANCE SUGGESTED THEY LAND IN ELP. AIRCRAFT WAS HALF WAY TO KELP AND IT WOULD HAVE TAKEN THE SAME TIME TO RETURN TO CNM. MAINTENANCE OPENED THE TAIL WAS OPENED TO CHECK FOR LEAKS. TEMPS WERE NORMAL WHEN THE COMPARTMENT WAS ACCESSED. A LEAK IN THE BLEED AIR SYSTEM WOULD'VE MADE THE TAIL SECTION VERY WARM/HOT. AFTER TROUBLESHOOTING BLEED AIR SYSTEM, SYSTEM INCORPORATES A RESET OF SOME ITEMS WHICH IS THE TAIL CONE. MAINTENANCE LIGHT SWITCH & IS PART OF THE MAINTENANCE WARNING PANEL IN THE TAIL CONE. THE GROUND WIRE ON THE MAINTENANCE LIGHT SWITCH HAD COME LOSE AND TOUCHED THE POWER WIRE ACTIVATING THE BLEED LIGHT WARNING ANNUNCIATORS. THE GROUND WIRE WAS REPAIRED AND THE MAINTENANCE LIGHT CURRENT LIMITER REPLACED. THE BLEED AIR LIGHTS RESET, BOTH ENGINE WERE THEN RUN UP TO FULL POWER AND A LEAK CHECK OF THE TAIL SECTION BLEED AIR LINES COMPLETED, THERE WERE NO LEAKS AND NO BLEED LIGHTS ILLUMINATED. THE AIRCRAFT WAS RETURNED TO SERVICE.
#1 (LEFT) MAIN BOOST PUMP FAILED
#2 (RIGHT) HYDRAULIC PUMP FAILURE.
CLIMBED TO 200 FT AGL, BROUGHT GEAR UP. BEFORE GEAR UP, LIGHTS OUT 400 FT, HEARD A NOTICEABLE INCREASE IN NOISE FROM ENG AREA & NOTICED A SURGE & A MASTER CAUTION LIGHT, EIS CAUTION LIGHT & TOP ROW OF EIS FLASHING. RETURNED TO AIRPORT. MAXIMUM TORQUE OBSERVED WAS AROUND 50.8 TO 51 PERCENT, ITT MAXIMUM TEMP OBSERVED WAS AROUND 870, NG AND FUEL FLOW WERE NOT OBSERVED. OIL TEMP AND PRESSURE WERE NORMAL AND THE COPILOT HAD CHECKED THE EIS AT 80 KTS AND WAS OBSERVED TO BE NORMAL.
