RIGHT THROTTLE FAILED TO ADVANCE ON TAKEOFF. FOUND BROKEN RIGHT THRUST REVERSER AFT INTERLOCK FEEDBACK CABLE FLEXIBLE CASING ASSEMBLY.
RIGHT #4 CABIN WINDOW FAILED IN FLIGHT, OUTER PANE SHATTERED SENDING DEBRI INTO THE RIGHT ENGINE INLET, THE RIGHT ENGINE NOSE COWL WAS DAMAGED, 20 LOW PRESSURE FAN BLADES DAMAGED, BORESCOPE INSPECTION DETERMINED NO DAMAGE TO CORE ROTATING GROUP.
CREW REPORTED FLAPS INOPERATIVE, STUCK AT 7 DEGREES. FAILED FL23 CURRENT LIMITER, REPLACED FL23 CURRENT LIMITER P/N ANL4. INVESTIGATED REASON FOR FAILED CURRENT LIMITER AND FOUND SEVERAL FLAP ACTUATORS SCREW SHAFTS TO BE ROUGH AND POSSIBLY BINDING UNDER A LOAD, AND ONE ACTUATOR ON THE LEFT WING THAT HAD A VERY WORN SCREW SHAFT THAT WAS CAUSING THE YOKE TO BIND ABOUT THE AREA THAT THE FLAPS WOULD BE ABOUT SEVEN DEGREES. REMOVED ALL FLAPS AND INSPECTED ALL COMPONENTS FOR CONDITION AND WEAR. CLEANED AND INSPECTED ALL COMPONENTS AND REASSEMBLE WITH SERVICEABLE ACTUATORS AND YOKES.
PILOT REPORTED THE RIGHT GREEN GEAR LIGHT ILLUMINATED WHEN THE GEAR WAS UP DURING CLIMB. CAUSE: FOUND BROKEN WIRE AT PIN "C" OF THE UPLOCK SWITCH JW8. THIS WAS GIVING A FALSE INDICATION THAT THE RIGHT GEAR WAS ALREADY UP AND CAPTURED WHICH DISCONTINUED HYDRAULIC PRESSURE TO THE RIGHT GEAR BEFORE IT WAS ACTUALLY ALL THE WAY UP ALLOWING THE GEAR TO FREE FALL TO THE DOWN AND LOCKED POSITION. THEREFORE CAUSING THE RIGHT GEAR GREEN LIGHT TO ILLUMINATE. CORRECTIVE ACTION: REPLACED CONNECTOR P/N: CTJ620E06N AND FUNCTION THE LANDING GEAR SYSTEM WITH REFERENCE TO THE CITATION 650 AMM REV 38 SECTION 32-00-01-06 REV 33. OPERATIONAL CHECKED SATISFACTORY WITH NO OTHER DEFECTS NOTED.
WHILE ON CLIMB OUT AND TEN MINUTES INTO FLIGHT GOT HYDRAULIC VOLUME LOW ANNUNCIATOR. CREW VERIFIED THE VOLUME LOW AND PERFORMED THE HYDRAULIC VOLUME LOW CHECKLIST, PILOT DECLARED AN EMERGENCY AT KHOU AND RETURNED AND LANDED THE AIRCRAFT FURTHER INCIDENT. POSITIONED THE AIRCRAFT TO MAINTENANCE HANGER AND UPON INVESTIGATION FOUND THAT THE HYDRAULIC RESERVOIR WAS DEPLETED DUE TO A RUPTURED HYDRAULIC HOSE JUST OUTBOARD OF THE RIGHT MAIN GEAR GOING TO THE DOWN PORTION OF THE RIGHT MAIN GEAR ACTUATOR. ALL GEAR CYCLED DOWN AND LOCKED UTILIZING THE NORMAL HYDRAULIC SYSTEM, DID NOT NEED TO UTILIZE THE EMERGENCY BLOW DOWN SYSTEM BUT THE PILOT DID USE THE EMERGENCY BRAKE SYSTEM DUE TO THE UNRESPONSIVE BRAKE PEDALS.
GEAR NOT LOCKED LIGHT ILLUMINATED. FOUND LEFT UPLOCK ROD ASSEMBLY WOULD NOT EXTEND AND ALLOW HOOK TO FULLY CAPTURE ROLLER ON LOWER STRUT ASSEMBLY. CLEANED INSPECTED AND UPLOCK ROD ASSEMBLY, CYCLED GEAR SEVERAL CYLES ON JACKS AND FOUND OPERATION SATISFACTORY.
DURING SETUP FOR APPROACH TO LANDING STALL, RIGHT ENGINE FAILED AND WAS SECURED. EMERGENCY WAS DECLARED AND AIRCRAFT RETURNED TO DEPARTURE WITH NO FURTHER INCIDENT. MAINTENANCE DISCOVERED THAT THE WERE LITTLE TO NO CONTROL OF THE PROPELLER AND AFTER INVESTIGATION FOUND THAT THE TEETH ON THE GOVERNOR DRIVE GEAR HAD SHEARED.
PRIMARY TRIM FAILED IN FLIGHT. IAW CHECKLIST SWITCH TO SECONDARY TRIM. SECONDARY TRIM FAILED. ACCOMPLISHED DUAL TRIM FAILURE CHECKLIST. DECLARED AN EMERGENCY AND LANDED WITHOUT ANY FURTHER INCIDENT.
AIRCRAFT HIT A BIRD ON APPROACH DURING FAR 61/91 TRAINING. CREW ABANDONED APPROACH AND RETURNED WITH NO PROBLEMS. FOUND BENT RIGHT PITOT TUBE AND 1 LANDING/TAXI LIGHT BROKEN.
HAD NO NOSE GEAR INDICATION ON INITIAL APPROACH TO LAND. WENT AROUND AND CYCLED THE GEAR MULTIPLE TIMES. FLEW BY THE TOWER TO GET A VISUAL INSPECTION, THEY SAID THE GEAR APPEARED TO BE DOWN. THEN WE PERFORMED THE EMERGENCY GEAR EXTENSION CHECKLIST. STILL HAD NO NOSE GEAR LIGHT. DID ANOTHER FLYBY OF THE TOWER, THEY SAID IT WAS DOWN SO WE ATTEMPTED A LANDING TO FEEL THE NOSE GEAR, IT WAS DOWN, LANDED AND PARKED WITH NO FURTHER INCIDENT.
PRIMARY TRIM CHECKED GOOD ON PREFLIGHT. ON DEPARTURE PRIMARY TRIM FAILED. SELECTED TO SECONDARY TRIM IAW THE CHECKLIST, DECLARED AN EMERGENCY AND RETURNED TO DEPARTURE WITHOUT INCIDENT.
ON CLIMBOUT LT ENGINE FIRE LIGHT CAME ON, PERFORMED PRECAUTIONARY ENGINE SHUTDOWN CHECKLIST, RETURNED TO DEPARTURE AIRPORT AND LANDED WITHOUT ANY INCIDENT. MX FOUND A FAULTY FIRE DETECTOR.
NO GREEN LIGHT INDICATION ON LT MAIN, JACKED ACFT AND FOUND GEAR FULLY DOWN AND SECONDARY LOCK NOT FULLY ENGAGED TO MAKE THE GEAR LOCKED SWITCH. REMOVED SECONDARY LOCK ASSY FOR INSPECTION. FOUND NO MECHANICAL DEFECTS. CLEANED AND LUBRICATED ALL PARTS AND REASSEMBLED SECONDARY LOCK. CONDUCTED MULTIPLE GEAR SWINGS WITH NO DISCREPENCIES AND RETURNED ACFT TO SERVICE.

FOUND NO MECHANICAL DEFECTS. CLEANED AND LUBRICATED ALL PARTS AND REASSEMBLED SECONDARY LOCK. CONDUCTED MULTIPLE GEAR SWINGS WITH NO DISCREPENCIES AND RETURNED ACFT TO SERVICE.
ON DESCENT FLIGHT CREW NOTICED THAT THE LEFT OIL PRESSURE GAUGE WAS GETTING INTO THE YELLOW ARCH AND A FEW MINUTES LATER THE OIL PRESSURE ANNUNCIATOR BEGAN TO FLICKER. PERFORMED PRECAUTIONARY ENGINE SHUTDOWN CHECKLIST TO AVOID DAMAGING THE ENGINE AND LANDED AT THE SCHEDULED STOP. A CERTIFIED REPAIR STATION CONFIRMED THAT THE ENGINE WAS LEAKING OIL FROM THE CORE DUE TO HIGH CASE PRESSURES IN ALL ENGINE ZONES POSSIBLY AS A RESULT OF A FAILED 4 OR 5 CARBINE SEAL.
DURING CRUISE AT 7000FT, THE RIGHT ENGINE BEGAN RUNNING ROUGH AND LOOSING POWER. THE ENGINE STALLED OUT AND COMPLETELY SHUT DOWN. FOLLOWED AIRCRAFT CHECKLIST AND ATTEMPTED TO RESTART THE ENGINE UNSUCCESSFUL. SECURED FAILED ENGINE ACCORDINGLY TO THE AIRCRAFT CHECKLIST. DECLARED AN EMERGENCY AND MADE AN UNSCHEDULED LANDING AT THE NEAREST AIRPORT WITH OUT ANY FURTHER INCIDENTS OR INJURIES.
ON CLIMB-OUT RIGHT ENGINE FIRE LIGHT CAME ON. PERFORMED PRECAUTIONARY ENGINE SHUT DOWN CHECKLIST, RETURNED TO DEPARTURE AIRPORT AND LANDED. MAINTENANCE FOUND THAT THE LIP HEAT TUBE CAME OFF IN FLIGHT POSSIBLY DUE TO RUBBING ON COWLING. DUCT WAS FITTED TO EXHAUST STACK TUBE AND SECURED WITH NEW CLAMP. FIRE DETECTION SYSTEM GROUND CHECKED SERVICABLE WITH MAINTENANCE CHECKS AND PERFORMANCE RUNS. AIRCRAFT RETURNED TO SERVICE.
ON CLIMBOUT RIGHT ENGINE FIRE LIGHT CAME ON. PERFORMED PRECAUTIONARY ENGINE SHUTDOWN CHECKLIST AND LANDED AIRCRAFT. MAINTENANCE FOUND RIGHT ENGINE INBOARD EXHAUST STACK LIP HEAT TUBE TO BE CRACKED. DAMAGED EXHAUST STACK REPAIRED BY OUTSIDE VENDOR AND INSTALLED ON AIRCRAFT. AIRCRAFT TESTED SERVICABLE AND RETURNED TO SERVICE.
ON APPROACH, LANDING GEAR WAS SELECTED DOWN AND THERE WAS NO INDICATION OF GEAR BEING DOWN AND LOCKED (GREEN LIGHTS). THE GEAR HAD APPEARED TO TO ACT AND SOUND NORMAL DURING EXTENSION. CIRCUIT BREAKERS WERE CHECKED AND THE GEAR WAS CYCLED UP AND INDICATED BOTH MAIN GEAR DOWN AND LOCKED. GEAR WAS CYCLED BACK DOWN AND NO GEAR INDICATION WAS APPARENT. ACFT DIVERTED AND GEAR WAS VERIFIED WITH TAXI LIGHT OPERATION ON MAIN GEAR AND VISUAL CHECK BY TOWER PERSONAL. ACFT LANDED AND WAS INSPECTED BY EMERGENCY CREW AND RETURNED TO HANGAR FOR INSPECTION AND REPAIR.
LT HYD PRESSURE LOW LIGHT ILLUMINATED, INDICATING HYD PRESSURE BELOW 1400 PSI AT LT ENG DRIVEN HYD PUMP, SYS PRESSURE REMAINED NORMAL AT 2000 PSI, AS CREW MONITORED SYS PRESSURE THE RT HYDR PRESSURE LOW LIGHT ILLUMINATED, CREW AT THIS POINT DIVERTED TO MSP FOR RUNWAY LENGTH REQUIREMENTS. ACFT LANDED WITHOUT INCIDENT AT MSP. UPON INSP, THE MAIN HYD SYS FILTERS WERE LEAKING AND DETERMINED TO HAVE FAILED O-RINGS. O-RINGS WERE REPLACED, HYD SYS SERVICED AND PERFORMED LEAK CHECK AND LANDING GEAR FUNCTIONAL TEST IAW MM 32-30-00. ACFT RETURNED TO SERVICE.
