NTSB accidents & incidents
Operated by CJ Systems Aviation Group, Inc. (per NTSB report)
The loss of engine power during cruise flight due to third stage turbine wheel blade fatigue and subsequent separation, which resulted in an emergency landing.
Operated by CJ Systems Aviation Group (per NTSB report)
The pilot's inadequate preflight preparation, which resulted in the cyclic stick lock not being disengaged prior to lift-off, and his subsequent inability to control the helicopter. Contributing to the accident was the operator's inadequate procedures, the unmarked cyclic lock, and the excessive breakout force required.
Operated by CJ Systems Aviation Group (per NTSB report)
The pilot's failure to maintain main rotor rpm and proper descent rate resulting in a hard landing. A related factor in this accident was inadvertent encounter with clouds.
Operated by CJ Systems Aviation Group (per NTSB report)
Loss of control for an undetermined reason during maneuvering flight, which resulted in an in-flight collision with water.
Operated by CJ Systems Aviation Group (per NTSB report)
The loose tail rotor drive shaft coupling due to its improper installation by the operator's maintenance personnel, which resulted in the failure of the tail rotor drive shaft. An additional cause was the inability of the pilot to maintain control of the helicopter in the hover following the drive shaft failure.
Operated by CJ Systems Aviation Group, Inc. (per NTSB report)
The failure of the pilot to maintain rotor rpm and his inadequate recovery from a bounced landing. Factors contributing to the accident included the tailwind condition and the tree.
Operated by CJ Systems Aviation Group (per NTSB report)
The sequential non-mechanical total loss of power in both engines 1 and 2 for undetermined reasons and the pilot's failure to maintain adequate rotor RPM to prevent a hard landing. Contributing factors were the tripped "RPM WARN" circuit breaker which disabled the engine failure and low rotor warning lights and aural warnings.
Operated by CJ SYSTEMS AVIATION GROUP, INC (per NTSB report)
pilot's failure to maintain directional control of the helicopter while hovering, which resulted in its collision with a building. Factors were the confined area, tail wind, and wind gusts.

Maintenance disclosure history
No maintenance difficulty reports on file. Note: SDR filing is largely voluntary, so absence is not evidence of a clean maintenance history.
