WHEN DECENDING FROM 14K FEET TO 12K FEET, CREW ACTIVATED SPD MODE TO ALLOW CONTROL RATE VIA ENGINE POWER. ONCE ENGINE TORQUE WENT BELOW 10PSI CREW HEARD LOUD BANG FROM FRONT OF AIRCRAFT AND A THUD WAS FELT. NO ENGINE INSTRUMENTS INDICATING ANYTHING WAS WRONG AND NO CASS MESSAGE.
ON JULY 23, 2025, AT APPROXIMATELY 19:47 LOCAL TIME, FLIGHT 807, OPERATING AIRCRAFT N874SA, EXPERIENCED A LEFT-HAND MAIN LANDING GEAR (LH MLG) TIRE FAILURE DURING THE LANDING ROLLOUT. AS A RESULT OF THE TIRE BLOWOUT, THE AIRCRAFT CAME TO A STOP ON THE TAXIWAY. THE FLIGHT CREW COORDINATED WITH SIGNATURE AVIATION FOR ASSISTANCE, AND THE AIRCRAFT WAS SUBSEQUENTLY TOWED TO THE SIGNATURE AVIATION RAMP, CLEARING THE TAXIWAY WITHOUT FURTHER INCIDENT.
ON CLIMB OUT FROM KVNY, PILOTS GOT A PRIM STAB TRIM FAIL, MACH TRIM FAIL, RUDDER LIMITER FAIL, CAS MESSAGES. AFTER ABOUT 5 SECONDS, ALL BUT PRIM STAB TRIM FAIL MESSAGES WENT AWAY AND COULD NOT BE CLEARED. PILOTS REVERTED TO SEC TRIM AND DECIDED TO RETURN TO KVNY SINCE THE AIRCRAFT IS ALTITUDE AND SPEED LIMITED WHEN USING SECONDARY TRIM. LANDED WITHOUT INCIDENT. MECHANIC WAS DISPATCHED TO T/S THE PROBLEM. WHEN HE POWERED UP THE PLANE, THE CAS MESSAGE WAS GONE AND THE TRIM SYSTEM RAN NORMALLY. RE-RACKED THE HSTECU AND PERFORMED FUNCTIONAL TEST PER THE M.M. NO DEFECTS NOTED. AIRCRAFT RETURNED TO SERVICE.
THE AIRCRAFT HAD AN ELEVATOR TRIM RUNAWAY ON ONE LEG, RESET AND FLEW FINE THE SECOND LEG. THIRD LEG HAD A RUNAWAY RUDDER TRIM.
FIRE DETECTOR CAS MESSAGE IN FLT. RETURNED TO DEPARTURE. MAINTENANCE TROUBLESHOOTING.
FROM PIREP ON LOGPAGE 3222 ENG CHIP CAS + ENGINE VIBRATION ON DEPARTURE PASSING THROUGH 800' OFF RWY 25 @ KSBA OIL TEMP & OIL PX WERE NORMAL SF4766 MAINTENANCE TROUBLESHOOTING IN PROGRESS. WILL UPDATE.
SMOKE COMING FROM THE REAR ELECTRICAL HATCH. AIRCRAFT WAS ON GROUND. TROUBLESHOT SMOKE COMING FROM REAR ELECTRICAL HATCH. FOUND VCCS MOTOR INTERNALLY SHORTING OUT. PERFORMED ENGINE GROUND RUN UPS. INSPECTED REAR BAY CANNOT FIND ANY FURTHER ISSUES AT THIS TIME. REMOVED THE VCCS AND REPLACED WITH A NEW UNIT, COMPLIED WITH SB 17-101.
UNDER VFR CONDITIONS ON A VISUAL APPROACH, THE FIRST OFFICER WAS HAND FLYING. AT APPROXIMATELY 1500FT AN AURAL TRIM RUNAWAY WARNING WAS ANNOUNCED FOLLOWED BY A MASTER WARNING PITCH TRIM RUNAWAY THAT WAS ILLUMINATED ON THE CAS SCREEN. THE GEAR WAS DOWN AND FLAPS WERE SET AT 15 PRIOR TO THE WARNING. THE CAPTAIN SWITCHED THE TRIM INTERRUPT SWITCH AND TOOK OVER CONTROLS. ELECTED TO LAND THE AIRCRAFT AND FORGO THE QRH UNTIL ON THE GROUND. AN EMERGENCY WAS NOT DECLARED.
PITCH TRIM RUN-AWAY IN FLIGHT WITH OUT AUTO PILOT ENGAGED LEVELING OFF AT 15,000FT MSL.
PITCH TRIM RUN-AWAY IN FLIGHT WITH OUT AUTOPILOT ENGAGED LEVELING OFF AT 15,000FT MSL.
UPON CLIMBING THROUGH APPROXIMATELY 2500 FEET MSL IN A CLIMBING LEFT TURN DIRECT LAX VOR THE ENGINE NP CAST ILLUMINATED. IMMEDIATELY CALLED FOR THE QRH. MEANWHILE OBSERVED ENGINE NP IN EIS WINDOW RAPIDLY DECREASING ( INITIALLY 1650 THEN AS LOW AS 1300 RPM). DECLARED EMERGENCY AND REQUEST IMMEDIATE VECTOR TO RETURN TO HHR. WE WERE GIVEN A HEADING OF 090 AND DESCENT TO 2000 MSL. SLOWED THE AC TO 170 KIAS AND CALLED FOR GEAR EXTENSION FOLLOWED BY FLAPS 15, 30, AND THEN 40 DEGREES DUE TO OUT NEED TO LOOSE ADDITIONAL ALTITUDE WHILE ON HIGH FINAL. DURING THE DESCENT THE ENGINE NP CAS EXTINGUISHED AND ALL RPM/NP AND ENGINE INDICATIONS APPEARED NORMAL. LANDED WITHOUT INCIDENCE AND TAXIED BACK TO THE TERMINAL. NOTE, ALL ENGINE PARAMETERS AND PERFORMANCE WERE INDICATING NORMAL WITH THE EXCEPTION OF THE ENGINE NP AS DESCRIBED ABOVE.
ON APPROACH TO LANDING LT MAIN GEAR LIGHT WOULD NOT ILLUMINATE. A GO AROUND WAS INITIATED AND THE AIRCRAFT DEPARTED THE AIRPORT TRAFFIC AREA. NUMEROUS EMERGENCY GEAR EXTENSIONS WERE PERFORMED WITH THE SAME RESULT. A LANDING WAS ATTEMPTED WHILE PUMPING THE EMERGENCY EXTENSION HANDLE. ON ROLL OUT THE LT MLG LIGHT ILLUMINATED, NORMAL ROLL OUT FOLLOWED. ACFT WAS RETURNED TO MAINTENANCE WHERE THE DRAG LINK DOWN LOCK LEFT MLG WAS INSPECTED. THE DOWNLOCK PIN WAS FOUND LOOSE IN THE DOWNLOCK ASSY WITH THE RELATED SET SCREW BACKED OUT. THIS ALLOWED THE PIN TO ROTATE FREELY. A REVIEW OF O/H PROCEDURES INDICATED THE SET SCREW IS TO BE SECURED WITH THREAD SEALANT. NO SEALANT WAS OBSERVED, THE ASSY WAS DISASSEMBLED AND SEALANT APPLIED IAW THE STANDARD OPERATING PROCEDURES IN THE MM 32-10-00.
CREW REPORTED AFTER TAKE OFF, THEY OBSERVED PITCH TRIM RUNAWAY CAS MESSAGE ON THERE LOWER MFD SCREEN. RAN THE QRH PROCEDURE AND CALLED TOWER FOR EMERGENCY LANDING BACK TO DEPARTURE.
PITCH TRIM CIRCUIT BREAKER TRIPPED, AUTOPILOT FAIL CAWS ILLUMINATED.
PILOT REPORTED RUDDER TRIM RUNAWAY AFTER TAKEOFF DURING CLIMB OUT.
DURING FLIGHT, ACS IN-FLOW CAS MESSAGE AND CABIN PRESSURE WAS RELEASED IN FLIGHT. CREW DIVERTED.
CABIN DOOR SEPARATED FROM ACFT DURING TAKEOFF SEQUENCE FOR UNKNOWN REASONS. THE ACFT RETURNED TO AIRFIELD AND LANDED WITHOUT INCIDENT.
