AIRCRAFT LOST PRESSURIZATION AT FL450, AIRCRAFT DID EMEREGENCY DESCENT INTO KAQB, AIRCRAFT LANDING WITHOUT INCIDENT,
AIRCRAFT DEPARTED KADS ENROUTE TO KIAH, AT FL 250 PILOT NOTICED AIRCRAFT WAS HAVING PRESSURIZATION ISSUES, PILOT RETURNED BACK TO KADS, DECLARED AND EMERGENCY BUT CANCELED, AIRCRAFT LANDED WITHOUT INCIDENT. AIRCRAFT CURRENTLY IN MAINTENANCE.
DURING LANDING AIRCRAFT WAS ABOUT 50 FEET ABOVE THE GROUND JUST CROSSING RUNWAY THRESHOLD #2 ENGINE SHUTDOWN, AIRCRAFT LANDED WITHOUT INCIDENT, EMERGENCY WAS NOT DECLARED. WAITING ON TEXTRON AOG MAINTENANCE.
ON APPROACH INTO KABQ AT 7500 FT LH ENGINE SHUTDOWN WITH POWER SETTING AT 70% N1, PILOT DECLARED EMERGENCY, AIRCRAFT LANDED WITHOUT INCIDENT. WAITING ON TEXTRON AOG TEAM TO ARRIVE TO PERFORM INSPECTION.
AIRCRAFT DEPARTED KADS TODAY, AT FL300 CREW NOTICED RH ENGINE OIL PRESSURE WAS DROPPING, CREW ELECTED TO SHUTDOWN ENGINE AS A PRECAUTION, CREW DECLARED AN EMERGENCY AND LANDED BACK AT KADS WITH NO ISSUES.
CREW REPORTED ON FLIGHT FROM KADS DIRECT KMRF AT FL360 RH FIRE WARNING LIGHT ILLUMINATED, CREW FOLLOWED CHECKLIST AND WARNING LIGHT EXTINGUISHED, CREW DIAGNOSED WARNING LIGHT WAS FALSE. THEY CONTINUED ON WARNING LIGHT CAME ON AGAIN CREW DIVERTED TO KMAF. NO EMERGENCY DECLARED AND AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE HAS BEEN PERFORMED NO DEFECTS NOTED AND GROUND OPS CHECK OK. AIRCRAFT FLEW BACK TO KADS WITH NO RH FIRE WARNING LIGHT.
LH FIRE LIGHT CAME ON IN FLIGHT WENT OUT AFTER 15 SECONDS OWN ITS OWN AND DIDN'T REOCCUR. GAINED ACCESS TO HOT SECTION FIRE LOOP USED MEGGER TO CHECK FIRE LOOP, LOOP WAS ON RAGGED EDGE OF FAILING INSPECTION AS A PRECAUTION REPLACED LH HOT SECTION FIRE LOOP.
CREW REPORTED ON FLIGHT FROM KLAS DIRECT KEFD AT FL430 LH FIRE WARNING LIGHT ILLUMINATED, CREW FOLLOWED CHECKLIST AND WARNING LIGHT EXTINGUISHED, CREW DIAGNOSED WARNING LIGHT WAS FALSE. THEY CONTINUED ON WARNING LIGHT CAME ON AGAIN CREW DIVERTED TO KABQ. NO EMERGENCY DECLARED AND AIRCRAFT LANDED WITHOUT INCIDENT. I GOT ON SOUTHWEST AIRLINES FLEW TO ABQ, REMOVED LH ENGINE LOWER COWLING INSPECTED AND CLEANED ACCESSIBLE FIRE LOOPS, RAN ENGINES NO FIRE WARNING LIGHT. I FLEW BACK TO ADS ONBOARD AIRCRAFT FIRE LIGHT DID NOT ILLUMINATE DURATION OF TRIP. AS A PRECAUTION REPLACED LH ENGINE FIRE CONTROL UNIT.
CREW REPORTED CLIMBING THROUGH FL 150, RED REAR BAY OVERHEAT LIGHT ILLUMINATED, CREW FOLLOWED CHECKLIST PROCEDURES. LIGHT EXTINGUISHED WHILE STILL CLIMBING THROUGH FL 180 RED REAR BAY OVER HEAT LIGHT ILLUMINATED FOLLOWED BY A BURNING ODOR AND LIGHT SMOKE IN COCKPIT.
FLIGHT CREW REPORTED HEARING A POPPING SOUND FROM BEHIND THE COCKPIT LT SIDEWALL AREA FOLLOWED BY A SLIGHT ELECTRICAL ODOR FOR A PERIOD OF 15 SECONDS. NO SYSTEMS WERE AFFECTED AS ALL WERE OPERATING NORMALLY FOR THE DURATION OF THE FLIGHT (1.2 HRS). MFG AIRCRAFT SERVICES TECHNICIANS WERE DISPATCHED AND REMOVED ALL LT SIDEWALL PANELS FOR ACCESS. INSPECTION OF ALL ELECTRICAL COMPONENTS, CONNECTORS, AND WIRING REVEALED NO DEFECTS. SUBSEQUENT FLIGHTS WERE ALL UNEVENTFUL.
DURING CLIMBOUT, FLIGHT CREW REPORTED THE RT ENGINE N1 (FAN) OVERSPED TO 116.8 PERCENT. ITT DID NOT EXCEED LIMITS. N2 WAS RT AT RED LINE LIMIT OF 97.0 PERCENT. THE FUEL FLOW WAS EXTREMELY HIGH AT 1300 LBS/HR. FLIGHT CREW RETARDED POWER TO IDLE WITH NO CHANGE IN ENGINE OPERATING PARAMETERS. ENGINE WAS SHUTDOWN AND AIRCRAFT RETURNED AND LANDED SAFELY. SUSPECT FUEL CONTROL FAILURE. SUBSEQUENT FUEL CONTROL TEAR DOWN CONFIRMED THE DRIVE BODY BEARINGS FAILED WHICH RESULTED IN MAJOR INTERNAL DAMAGE.
HORIZONTAL STABILIZER TRIM WOULD NOT ACTUATE IN THE NOSE-DOWN TRAVEL DIRECTION (MAIN SYSTEM). STANDBY TRIM SYSTEM FUNCTIONED PROPERLY. FOUND THE MAIN CONTACTS OF THE -B- DOWN RELAY IN AN OPEN CIRCUIT CONDITION. REPLACEMENT CONFIRMED FAULT.
REPORTING AN ON-GROUND PROPELLER STRIKE INCIDENT THAT OCCURRED ON AIRCRAFT. THE AIRCRAFT WAS ON THE GROUND PERFORMEING PREFLIGHT CHECKS WHEN THE AIRCRAFT ROLLED OFF THE PAVED RUNUP AREA AND THE LT PROP STRUCK AN OBJECT. UPON PRELLIMINARY INSPECTION OF THE PROPELLER, 2 BLADES WILL REQUIRE REPLACEMENT. THE PROP HAS BEEN REMOVED AND FORWARDED TO A REPAIR FACILITY FOR OVERHAUL. THE ENGINE HAS BEEN INSPECTED IAW MM, AND WAS FOUND TO BE SERVICEABLE FOR CONTINUED OPERATION. (K)
NR 2 ENGINE EXPERIENCED AN INTERNAL HOT SECTION FAILURE RESULTING IN COMPLETE LOSS OF POWER AND FLAMEOUT. ALL ENGINE INDICATIONS WERE NORMAL PRIOR TO THE FAILURE. A ROUTINE HOT SECTION BORESCOPE INSPECTION HAD BEEN PERFORMED. 10.2 HOURS PRIOR TO FAILURE. THIS ENGINE IS 340.6 HOURS FROM SCHEDULED 4000 HOUR OVERHAUL. PRELIMIARY INSPECTION REVEALED COMPRESSOR TURBINES BLADES FAILURE IS PROBABLE GAUGE. (K)
