IN CRUISE FLIGHT THE CREW RECEIVED A FLAP ASYMMETRY WARNING WITH NO FLAP INPUT. THE CREW ELECTED TO DIVERT FROM THE ORIGINAL DESTINATION OF KMDW TO KORD FOR THE LONGER RUNWAY SINCE A NO FLAP LANDING WAS REQUIRED. AN EMERGENCY WAS DECLARED OUT OF AN ABUNDANCE OF CAUTION. THEY LANDED WITHOUT INCIDENT. MAINTENANCE REPORTED TO THE AIRCRAFT AND THE ASYMMETRY LIGHT WAS EXTINGUISHED AND THE FLAP SYSTEM OPERATED WITH NO FAULTS.
AFTER DEPARTURE UPON SELECTION OF GEAR UP THE LEFT MAIN LANDING GEAR REMAINED EXTENDED. TOWER FLYBY CONFIRMED SINGLE LEG EXTENSION. THE DECISION WAS MADE TO DIVERT TO LISBON PORTUGAL FOR BETTER AVAILABILITY OF SERVICES. THE AIRCRAFT DIVERTED AND LANDED WITHOUT FURTHER INCIDENT THOUGH AN EMERGENCY WAS DECLARED AS A PRECAUTION. UPON EXAMINATION OF THE LEFT MAIN LANDING GEAR, IT WAS NOTED THAT THE AFT SECTION OF THE INBOARD GEAR DOOR WAS HANDING DOWN SEVERAL INCHES. EXAMINATION REVEALED THAT THE HANGAR THAT THE SECURED THE GEAR DOOR ARM TO THE FUSELAGE HAD FRACTURED AT BOTH ATTACH LUGS.
PILOT REPORTED LEFT BRAKE LOSS ON TAXI IN TO RAMP. BRAKE CALIPER DEVELOPED A SEVERE LEAK AND WAS REPLACED.
AFTER TAKE-OFF AND GEAR UP SELECTION THE RED GEAR UNSAFE LIGHT REMAINED ILLUMINATED. THE CREW CYCLED THE GEAR WITH THE SAME RESULT. THEY RETURNED TO THE DEPARTURE AIRPORT AND ON GEAR DOWN SELECTION HAD THREE GREEN INDICATED WITH NO RED UNSAFE LIGHT. THEY LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE JACKED THE AIRCRAFT AND CYCLE THE LANDING GEAR AND FOUND NO ANOMALIES. THEY CLEANED AND TESTED THE GEAR UPLOCK SWITCHED AND NO FAULTS WERE FOUND.
UPON TOUCHDOWN THE PILOT REPORTED NO BRAKES, CO-PILOT ALSO REPORTED NO BRAKES, ANTI-SKID WAS SELECTED OFF AND BRAKING WORKED. A TIRE WAS FLAT-SPOTTED AND THE OTHER DEFLATED, AND THE AIRCRAFT HAD TO BE TOWED FROM THE RUNWAY. AFTER WHEELS WERE REPLACED, AIRCRAFT WAS TAXIED WITH ANTI-SKID ON AND OFF AND THERE WERE NO ISSUES. ANTI-SKID WAS DEFERRED, AND AIRCRAFT FLOWN TO MAINTENANCE FACILITY WHERE ANTI-SKID FUNCTIONAL CHECKS WERE PERFORMED WITH NO FINDINGS.
ON APPROACH TO LAND THE LEFT MAIN LANDING GEAR DOWNLOCK INDICATION FAILED TO ILLUMINATE. DECLARED EMERGENCY AND LANDED WITHOUT FURTHER EVENT.
AIRCRAFT DEPRESSURIZED IN CRUISE FLIGHT ACCOMPANIED BY NOISE FROM CABIN DOOR SEAL
DURING CLIMB NOSE LANDING GEAR EXTENDED UNCOMMANDED. AIRCRAFT RETURNED TO DEPARTURE AIRPORT AND LANDED WITHOUT INCIDENT. NOSE LANDING GEAR STRUT TO BE SERVICED.
ENGINE POWER REDUCED FOR DESCENT AND UPON LEVEL OFF ENGINE WOULD NOT ACCELERATE ABOVE FLIGHT IDLE. ENGINE WOULD NOT RESPOND IN MANUAL MORE. ENGINE SECURED AND AIRCRAFT LANDED WITHOUT FURTHER ISSUE. FUEL CONTROL REPLACED.
CREW REPORTED CASCADING ELECTRICAL FAILURES ON AIRCRAFT AND DIVERTED AND HAD EMERGENCY DECLARED FOR THEM. NO DISCREPANCIES WERE FOUND WITH AIRCRAFT. SUSPECT STARTER SWITCH WAS LEFT ON CAUSING RIGHT GEN TO NOT WORK AND THAT THE CREW SECURED THE LEFT GEN DUE TO HIGH LOAD AND MISINTERPRETATION OF CHECKLIST. MAINTENANCE INSPECTED GEN AND CURRENT LIMITERS WITH NO FAULTS FOUND.
ON ARRIVAL AT THE RAMP THE PILOT RIGHT BRAKE PEDAL WENT TO THE FLOOR AND WAS INEFFECTIVE. AIRCRAFT STOPPED USING LEFT BRAKE. BRAKE CALIPER WAS REMOVED AND SEALS REPLACED AND AIRCRAFT RETURNED TO SERVICE.
UPON ARRIVING AT THE RAMP THE RIGHT BRAKE FAILED AT CALIPER O-RING.
AFTER LANDING AND TAXI TO RAMP RIGHT BRAKE PEDAL WENT TO FLOOR WITH NO BRAKING ACTION. AIRCRAFT STOPPED USING LEFT BRAKE WITH NO FURTHER ISSUE. UPON INSPECTION FLUID WAS FOUND LEAKING FROM RIGHT BRAKE STAT-O SEAL. CALIPER REPLACED AND AIRCRAFT RETURNED TO SERVICE.
RIGHT ENGINE OIL SERVICE DOOR DEPARTED THE AIRCRAFT AND TORE A SECTION OF THE LOWER ENGINE COWL.
ON CLIMB CREW RECEIVED A DUCT OVERHEAT AND LOSS OF TEMPERATURE CONTROL. EMERGENCY PRESSURIZATION SYSTEM ACTIVATED AND CAUSED NOXIOUS ODOR IN THE CABIN. THE CREW HANDLED USING CHECKLIST AND LANDED WITHOUT FURTHER INCIDENT AT DESTINATION AIRPORT. MAINTENANCE IDENTIFIED THE AIR CYCLE MACHINE COOLING TURBINE HAD FAILED.
AFTER TAKEOFF UNUSUALLY HIGH PRESSURE REQUIRED TO KEEP THE AIRCRAFT UNDER PITCH CONTROL. EMERGENCY TRIM SELECTED AND TRIM CONTROL WAS REGAINED. AIRCRAFT RETURNED TO DEPARTURE AIRPORT AND LANDED WITHOUT FURTHER INCIDENT. ALL MAINTENANCE MANUAL CHECK WERE RUN AND PROBLEM WAS NOT ABLE TO BE DUPLICATED
WHILE TAXIING WE WERE PERFORMING OUR USUAL TAXI CHECKS. AFTER PERFORMING THE BRAKE CHECK AND BEGINNING A TURN TOWARDS RUNWAY THE BRAKE SYSTEM DID NOT DISENGAGE. WE WERE UNABLE TO TAXI THE AIRCRAFT. DURING TOWING OF THE AIRCRAFT TO THE RAMP THE LEFT BRAKE COMPLETELY LOCKED UP WHICH REQUIRED THE AIRCRAFT TO BE FIXED ON THE TAXIWAY. BRAKE ASSEMBLY REPLACED.
WHILE LANDING AT KPHL, THE NOSE LANDING LIGHT DID NOT ILLUMINATE AND A GO-AROUND WAS INITIATED. AFTER COMPLETION OF THE QRH, A LOW FLY-BY OVER THE RUNWAY WAS CONDUCTED AND THE CONTROL TOWER INDICATED THAT THE LANDING GEAR APPEARED TO BE IN THE DOWN POSITION. A NORMAL LANDING WAS MADE WITH NO FURTHER ACTION. MAINTENANCE WAS DISPATCHED TO THE AIRCRAFT AND UPON ARRIVAL THE CREW SAID THEY HAD TOUCHED THE ANNUNCIATOR AND IT BEGAN TO INDICATE BUT PRIOR TO THAT THE ANNUNCIATOR WOULD NOT TEST.
AIRCRAFT HAD A BRAKING ISSUE, REMOVED AND REPLACED THE BRAKE WITH A OVERHAULED UNIT AND BLED AS NECESSARY. OPS CHECKED GOOD AIRCRAFT RETURNED TO SERVICE.
THE AIRCRAFT SMELLED SMOKE, THE CREW LANDED THE AIRCRAFT IN MCI WITHOUT INCIDENT. MAINTENANCE WAS SENT TO THE AIRCRAFT AND REPORTED THAT THE SMELL CAME FROM THE #1 CDU UNIT. THE UNIT STARTED TO WORK BUT WE DECIDED TO REPLACE THE UNIT. THE UNIT IS AT COLLINS AND WE ARE AWAITING IT TO BE RETURNED FROM THE MAINTENANCE FACILITY.
AFTER TAKE OFF THE CREW NOTICED THAT THE CABIN CLIMBED WITH THE AIRCRAFT. THEY CYCLED THE GEAR SEVERAL TIMES WHILE BURNING FUEL FOR THE RETURN FLIGHT TO KBHM. THE AIRCRAFT LANDED WITHOUT INCIDENT AND THE MAINTENANCE DEAPTMENT IS TROUBLESHOOTING THE AIRCRAFT.
ON ROLL OUT THE AIRCRAFT SHOWED THAT THE OIL PRESSURE WAS LOW THE PILOT ABORTED AND RETURNED TO THE RAMP. UPON FURTHER INVESTIGATION THE CREW DETERMINED THAT AFTER AN ENGINE RUN ON THE RAMP THAT THE OIL PRESSURE WAS READING WITH I LIMITS AND CONTINUED THE TRIP.
ON APPROACH TO LANDING, THE CREW DID NOT RECEIVE DOWN AND LOCKED INDICATION FOR THE LT MLG. AN EMERGENCY WAS DECLARED AND THE CHECKLIST PROCEDURE WAS FOLLOWED. THE ACFT MADE AN UNEVENTFUL LANDING. MAINTENANCE DETERMINED THAT THE LAMP FOR THE LT MLG DOWNLOCK INDICATION WAS INOPERATIVE.
DURING CLIMB OUT, AUTOPILOT STOPPED RESPONDING TO ALL PITCH COMMANDS. CREW RETURNED TO DEPARTURE. EMERGENCY DECLARED. MAINTENANCE FOUND A PIN ON THE CONNECTOR TO THE AUTOPILOT CONTROLLER NOT SEATED. LOCKING MECHANISM IN THE CONNECTOR FOR THAT PIN WAS BROKEN AND THE CONNECTOR TO THE CONTROLLER WAS REPLACED.
PILOTS REPORTED ENG SHUTDOWN DURING FLIGHT. MX REPORTED UPON INSP THAT THERE WAS A GRINDING SOUND INTERNAL TO THE ENG WHEN THEY TRIED TO ROTATE THE ENG BY THE STARTER. ENG WILL BE REMOVED AND SENT TO THE O/H SERVICE CTR FOR EVALUATION FOR REASON OF FAILURE.
UPPER SEAT BASE ASSY CRACKED AT CHAIR BACK ATTACH POINTS. STRESS ON CHAIR BACK AND METAL FATIGUE PROBABLE CAUSE. CHAIR WAS REPAIRED IAW PROCEDURESTC SA01042WI. (K)
LEFT ENGINE INFLIGHT SHUT DOWN, PILOTS REPORT LOSS OF TORQUE AND RISE IN ITT, ENGINE SHUT DOWN AND A SAFE LANDING COMPLETED.
INFLIGHT SHUT DOWN OF RIGHT ENGINE. TIME SINCE OH WAS 1210.00 HOURS.
GOODRICH BRAKE ASSEMBLY - WEAR PIN INDICATOR WAS JUST ABOUT AT POINT TO BE CHANGED. BRAKE WAS REMOVED TO CHANGE AND FOUND ONE BRAKE PLATE WAS CRACKED IN TWO PLACES, AND BROKEN. WHEN WHEEL WAS REMOVED, PARTS FELL OUT. SUBMITTER RECOMMMENDS 500 LANDING LIMIT ON BRAKE ASSY. PART CYCLES 562.

FLAP GEARBOX HAS AN OVERHAUL TIME OF 3,000 HOURS. SUBMITTER RECOMMENDS 1,000-HOUR OVERHAUL BE REQUIRED. THIS UNIT FAILED AT 1,650 HOURS.
FLAP MOTOR BURNS OUT WHEN GEARBOX FAILS AND TRIPS CIRCUIT BREAKER. THERE ARE TWO MOTORS AND TWO GEARBOX UNITS. WHEN GEARBOX FAILS, THE OPPOSITE MOTOR BURNS OUT TRYING TO CARRY LOAD.
FLAP GEARBOX INTERNAL GEAR STRIPPED OUT. SUBMITTER STATED THIS IS AN ONGOING PROBLEM. ALSO, WRITTEN UP IN CESSNA SERVICE MANUAL.
CVR TOTAL TIME ON UNIT SINCE LAST REPAIR IS 358 FLIGHT TIME. UNIT FAILS PUSH-TO-TEST FUNCTION.
INSPECTION FOUND BEARING IN FAN FAILED AFTER ONLY 84 HOURS SINCE NEW.
UNIT FAILED PUSH-TO-TEST. COCKPIT TEST ON PRE-FLIGHT. UNIT HAS 361 HOURS SINCE REPAIR. HIGH FAILURE RATE.
