RIGHT FLAP CABLE THAT ATTACHES TO THE OUTBOARD END OF FLAP, FOUND WITH BROKEN STRANDS, WHERE THE CABLE IS SWAGED TO THE TERMINAL THAT CONNECTS TO THE FLAP.
PILOT STARTED TO REDUCE POWER IN ORDER TO LOSE ALTITUDE. AT THIS TIME, THE THROTTLE CABLE BROKE CAUSING A LOSS IN ENGINE POWER. PILOT MADE SAFE LANDING IN A REMOTE FIELD. UPON INSPECTION, MAINTENANCE PERSONNEL FOUND THROTTLE CABLE BROKEN JUST ABOVE THE CABLE SUPPORT BRACKET. SUBMITTER SUSPECTED CAUSE AS IMPROPER HARDWARE USED ON SUPPORT BRACKET ALLOWING IT TO FLEX AS THROTTLE AND MIXTURE CONTROLS WERE CYCLED. SUBMITTER SUGGESTED TO PREVENT RECURRENCES, CLOSER INSPECTION OF FLIGHT CRITICAL ITEMS SHOULD BE PERFORMED IAW MANUFACTURER''S MM. (X)
FOUND GROUND STUD TO BE LOOSE ON BACK OF ALTERNATOR. (X)
OIL SUPPLY LINE FOR OIL COOLER, FITTING THREADS IN CASE DETERIORATED ALLOWING FITTING TO BLOW OUT. THIS ALLOWED ENGINE OIL TO PUMP OUT. PILOT SHUT ENGINE DOWN WITHOUT DAMAGE TO ENGINE. (X)
UPON INSPECTION OF PLUGS, FOUND PLUGS, 4 EACH, TO TEST WEAK AND NOT APPROPRIATE FOR INSTALLATION ON AIRCRAFT. NOTE: BY DOM AIR CARRIERS, ONE OF THE FOUR SPARK PLUGS HAD A RED DISCOLORATION AT BASE OF ELECTRODE AND CERAMIC. LOT NR 4G99.
SPARK PLUGS WERE REPLACED BECAUSE OF WEAR. UPON INSPECTION OF NEW PLUGS, FOUND WIRE WRAPPED AROUND CENTER ELECTRODE ON TWO OF THE NEW PLUGS (WHICH GROUNDED IT OUT) AND ANOTHER SIX WOULD NOT TEST GOOD IAW CHAMPION TEST PROCEDURES. OBTAINED ANOTHER NEW SET OF PLUGS AND FOUND ONE LUG WITH WIRE AROUND CENTER ELECTRODE AND TWO OTHERS WOULD NOT TEST GOOD. BOTH SETS CAME FROM THE SAME BATCH OR LOT. LOT NR 4G99.
INSTALLED NEW PLUGS IN ENGINE. PERFORMED RUN-UP. OPS OK. FOUR HOURS LATER, ENGINE HAD A 600 RPM DROP. REMOVED SPARK PLUGS, FOUND A PIECE OF WIRE AROUND CENTER ELECTRODE OF 3 PLUGS AND 4 OTHER PLUGS NOT FIRING VERY HOT ON PLUG TESTER. OPENED ANOTHER SET OF LUGS AND FOUND PIECE OF METAL RING AROUND CENTER ELECTRODE. APPEARED TO BE FROM MANUFACTURING PROCESS. LOT NR 4G99.
BEARING HAD LOST LUBRICANT AND STARTED TO CEASE. ONE STYLE BEARING HAS PROVISION FOR A LUBE FITTING, P/N 408420072, 1.31 INCH DIAMETER TRUNNION. THIS DIAMETER BEARING 1.19 INCH REQUIRES REMOVAL OF THE NOSE GEAR TO LUBE.
COLLAR WORK LOOSE OR BREAKS. THIS LETS TIRE ROTATE AND GROUND CONTROL OF AIRCRAFT IS LOST. SUBMITTER STATED THIS PROBLEM IS THE THIRD IN 3 YEARS. ONE SUCH BREAK CAUSED DAMAGE TO AIRCRAFT. WHEN IT WENT OUT OF CONTROL ON LANDING ROLL-OUT.
DURING AAIP INSPECTION, LEFT WING, LOWER SIDE, AT THE GEAR ATTACHMENT POINT, WAS FOUND WITH A CRACK. THIS CRACK IS BEYOND THE ALLLOWABLE LIMITS AS SPECIFIED BY THE AEROSTAR SM AND SB 600-66.
AT LANDING PILOT NOTICED OIL PRESSURE LOW. DURING TAXI, PILOT SHUT ENGINE DOWN. MAINTENANCE FOUND ENGINE WOULD NOT TURN BY PROP MOVEMENT. ENGINE WAS REMOVED AND SENT FOR TEARDOWN INSPECTION. THIS INSPECTION REVEALED THAT TORSION SHAFT HAD SEIZED DUE TO SCAVANGE PUMP FAILURE.
DURING AAIP INSPECTION, LEFT WING, LOWER SIDE, AT THE GEAR ATTACHMENT POINT, WAS FOUND WITH A CRACK. THIS CRACK IS BEYOND THE ALLLOWABLE LIMITS AS SPECIFIED BY THE AEROSTAR SM AND SB 600-66.
JAM NUT HOLDING YOKE TO PEDAL BACKED OFF LETTING SHAFT ROTATE AND COME APART. WHEN ROD END (YOKE) CAME OFF OF BRAKE ACTUATOR, BRAKING ACTION WAS LOST ON LEFT BRAKE. PILOT WAS ABLE TO PARK AIRCRAFT USING STEERING AND LOWER POWER FOR TAXI. RETURN SPRING, YOKE, AND JAM NUT WERE RE-INSTALLED AND AIRCRAFT RETURNED TO SERVICE.
DURING A DISASSEMBLY INSPECTION OF PROPELLER LINK, THE BLADE ACTUATING ON THE NR 2 BLADE WAS BROKEN AT THE .4375 INCH BLADE BUTT ATTACH POINT AND THE CHROME PLATING ON THE BLADE PITCH FITTING WAS DETERIORATED AND SHOWED EXTENSIVE PITTING. SUSPECT POSSIBLE FAILURE TO REPLACE OR REWORK COMPONENTS DURING OVERHAUL. PHOTOS AVAILABLE UPON REQUEST.
DURING TAKEOFF ROLL, AIRCRAFT VEERED TO THE RIGHT. PILOT ABORTED TAKEOFF BRAKING TO KEEP AIRCRAFT ON RUNWAY. PILOT GOT AIRCRAFT STOPPED WITH NO OTHER PROBLEMS. INVESTIGATION REVEALED THE STEERING SWITCH FAILED AND WAS STUCK IN THE RIGHT TURN POSITION AND WOULD NOT SPRING BACK TO THE CENTER OFF POSITION. REPLACED SWITCH, OPS CHECK GOOD. SWITCH SHOULD BE CHECKED FOR GOOD STRONG POSITIVE SPRING BACK TO THE CENTER OFF POSITION.
ATTACH POINT FOR UPPER TORQUE LINK FOR MLG BAND, WHERE LINK ATTACHES TO STRUT CRACKED AND BROKE AT LANDING. TIRE TURNED SIDEWAYS, AND AIRCRAFT DEPARTED THE RUNWAY AND HIT VASI LIGHTS CAUSING BOTH GEARS TO COLLAPSE. TTAF, 7,928.7 HRS.
