Part: IMPULSE COUPLING — DEFECTIVE (MAGNETO) · airframe 14,468 hrs · 10,000 cycles
REMOVED MAGNETO MODEL 6351 FOR THE 250 HOUR IMPULSE INSPECTION REQUIRED BY AD 99-04-04. THE MAGNETO IMPULSE COUPLING FAILED THE INSPECTION SO WE INSTALLED A SERVICEABLE IMPULSE COUPLING. THE MAGNETO HAD 198.8 HOURS SINCE NEW. 16 HOURS LATER THE MAGNETO MODEL 6351, SN 14110907 WAS HARD STARTING AND REPLACED WITH A NEW MAGNETO MODEL 6351, SN 15080112. MAGNETO MODEL 6351 WAS REMOVED FOR SCHEDULED 250 HOUR AND HAD BECOME HARD STARTING SO I REQUESTED THE 250 IMPULSE INSPECTION AND ALSO THE 500 HR INSPECTION. THIS MAGNETO IMPULSE COUPLING ALSO FAILED THE INSPECTION HAVING LOOSE RIVETS. THIS MAGNETO HAS 244.4 HOURS SINCE NEW.
Part: CYLINDER HEAD — CRACKED (ENGINE) · airframe 9,333 hrs
OUR ACFT DEPARTED WITH ONLY THE PILOT ON BOARD. THE ENG LOST PARTIAL POWER SO HE RETURNED. UPON INVESTIGATION CYLINDER 2 HAD NO COMPRESSION & WAS CRACKED HALF WAY AROUND THE CYLINDER HEAD. CYLINDER 4 WAS FOUND TO HAVE SOMEWHAT NORMAL COMPRESSION BUT DID INDICATE A LEAK & SMALL CYLINDER HEAD CRACK ALSO. IAW MFG TECH SERVICES, REPLACED ALL FOUR CYLINDERS WITH NEW MFG KITS.

UNABLE TO CONTROL ENGINE WITH POWER LEVER. ACCESS TOTAL INFLIGHT FEATHERING. FUEL CONTROL LINKING ROD END SEIZED, PREVENTED OPERATION OF FCU.

FUEL LEAK IN AREA OF FUEL PUMP ABOVE INJECTOR. CAUSED FIRE. HOSES WERE NOT REMOVED TO DETERMINE EXACT CAUSE.
Part: IDLER SHAFT — SCREW MISSING (PROP GOVERNOR)
WHILE PERFORMING A ROUTINE OIL CHANGE ON A PIPER PA32301, A 10-32 SET SCREW WAS FOUND IN THE OIL SUMP SUCTION SCREEN. THE SCREW HAD OBVIOUSLY BEEN SMASHED AND WAS IN TWO PIECES. INVESTIGATION FOUND THE ONLY 10-32 SET SCREW IN THE IO-540 SERIES ENGINES WAS USED TO HOLD THE PROP GOVERNOR IDLER GEAR SHAFT IN THE FRONT OF THE CASE. AFTER REMOVING THE PROPELLER AND THE EXTERIOR CAP, PLACED A MAGNET ON THE IDLER SHAFT AND WAS ABLE TO NEARLY PULL IT OUT OF THE CASE. NOT KNOWING THE EXTENT OF DAMAGE INTERNALLY AND SINCE THE ENGINE WAS NEAR OVERHAUL TIME, THE OWNER OPTED TO REPLACE THE ENGINE WITH A FACTORY OVERHAULED ENGINE.
