PILOT'S "A" SCREEN OUTER LAYER CRACKED AT FL370. EMERGENCY WAS DECLARED AND AIRCRAFT DIVERTED TO KRDU. LANDING WAS SUCCESSFUL WITH NO FURTHER ISSUE.
PILOTS REPORT "REAR BAY OVERHEAT" ANNUNCIATOR ILLUMINATED DURING CRUISE FLIGHT. DESCENT WAS REQUESTED BUT NOT AUTHORIZED BY ATC, SO AN EMERGENCY WAS DECLARED IN ORDER TO DESCEND. WARNING LIGHT EXTINGUISHED A SHORT TIME AFTER DESCENT WAS COMPLETED. INSPECTION OF THE VALVE REVEALED A SEVERE WARP AT THE FLANGE CONNECTING THE LP AND HP DUCTS FROM THE ENGINE. SEVERAL STUDS WERE SHEARED AT FLANGE.
AS REPORTED BY PILOTS: "WE WERE MEDEVAC STATUS DEPARTING PHL BOUND FOR YIP WITH 3 PAX ON BOARD. NORMAL ENGINE START AND TAXI FROM ATLANTIC FBO RAMP TO APPROACH END OF RWY35. CLEARED FOR TAKEOFF RWY35. AS I MOVED THE POWER LEVERS FORWARD TO SET TAKEOFF POWER, JUST ABOUT AT THE END OF TRAVEL, I NOTICE THE AIRCRAFT VEERING STRONGLY TO THE RIGHT. WHEN IT BECAME CLEAR THAT I HAD LIMITED DIRECTIONAL CONTROL OF THE AIRCRAFT, I BROUGHT THE POWER LEVERS BACK TO IDLE, AND ABORTED THE TAKEOFF. AIRSPEED WAS WELL BELOW 40KTS AT THE TIME OF ABORT. WE EXITED THE RUNWAY AND REQUESTED TAXI BACK." ADDITIONALLY, PILOTS REPORTED THAT THE R/H ENGINE HAD SHUT DOWN UPON APPLICATION OF TAKEOFF POWER, NOTICING THE MWS ANNUNCIATORS DENOTING "LO OIL PRESSURE, LO FUEL PRESSURE, R GEN FAILURE, ETC..."
CREW EXPERIENCED A RIGHT ENGINE FIRE WARNING DURING CLIMB. PULLED POWER ON RIGHT ENGINE AND WARNING CEASED AFTER APPROXIMATELY 15 SECONDS. DIVERTED AND DECLARED AN EMERGENCY. INSPECTION FOUND NO EVIDENCE OF AN ENGINE FIRE AND THE HOT SECTION SENSING ELEMENT FAILED INSPECTION AND CHECK.
THE WHEEL HALF STEEL KEYWAY RIVET FAILED CAUSING THE KEYWAY TO BEND OUTWARD FROM THE CENTRIFICAL FORCE, AND THEN MADE CONTACT WITH THE TIRE SIDEWALL AND THE UPPER GEAR TORQUE LINK. THERE WAS DAMAGE TO THE TIRE SIDEWALL AND THE TORQUE LINK HAD MINOR SCORING.
BLEED AIR MIXING VALVE CYLINDER ASSY BLEW OFF OF ITS MOUNTING DUE TO FATIGUE AROUND MOUNTING SCREWS. WHEN THE CYLINDER BLEW OFF, IT TORE THE CONNECTOR PLUG OFF OF THE FUEL FLOW TRANSDUCER CAUSING FUEL FLOW INDICATION TO FAIL.
CREW NOTICED A LANDING GEAR DOOR UNSAFE INDICATION IN FLIGHT. THE GEAR WAS LOWERED WITH GEAR DOWN AND LOCKED INDICATIONS NORMAL. INSPECTION REVEALED THAT THE OB RT GEAR DOOR HINGE WAS CRACKED WHICH CAUSED THE DOOR TO NOT FULLY CLOSE. THE CONDITION WAS AGRIVATED BY HIGHT AIRSPEED. (K)
AT 10,000 FT, CREW OBSERVED A LANDING GEAR UNSAFE LIGHT, FOLLOWED BY VIBRATION AND AIR NOISE. AC WAS SLOWED TO BELOW 250 KTS AND THEN CREW HEARD A (BANG) NOISE. AFTER LANDING IT WAS NOTICED THA THE RT OB GEAR DOOR HAD SEPARATED FROM THE AIRCRAFT. (K)
THE UPPER LEFT THRUST REVERSER DOOR IS CRACKED APPROXIMATELY 11 INCHES. THE CRACK IS LOCATED BEHIND THE MIDDLE ROW OF RIVETS, APPROXIMATELY .5 INCH BEHIND THE RIVET CENTER LINE AND IS PERPENDICULAR TO LINE OF FLIGHT. THE DOOR HAS BEEN RETURNED TO THE MFG FOR REPAIR.
EFD FAILED, CAUSING SMOKE IN COCKPIT DURING FLT. CRT WENT BLACK, SPECIFIC PART THAT BURNED IS UNKNOWN.
ON APPROACH, LANDING GEAR WAS LOWERED AND CDS MESSAGE LEFT AND RIGHT INBOARD BRAKE FAILED. CONTINUED WITH NORMAL CHECKLIST. THE AMBER MESSAGE WAS REPLACED BY A WHITE MESSAGE. RETURNED TO BASE AND DOWN LOADED COMPUTER SYSTEMS, OPERATED NORMAL. SENT FINDINGS TO LEAR ENGINEERING FOR STUDY.
CREW NOTICED SMOKE IN CABIN AFTER TAKEOFF AND RETURNED TO AIRPORT. MOTOR INSIDE BLOWER ASSY WAS FOUND HEAT AND SMOKE DAMAGED. (X)
DURING TAKEOFF ROLL, BOTH LEFT MAIN GEAR TIRES FAILED CAUSING DAMAGE TO BOTH LEFT MAIN WHEELS, BRAKES, AND OUTBOARD GEAR DOOR. BOTH TIRES WERE SHREDDED TO THE POINT THAT MADE ANY INSPECTION OF THE TIRES IMPOSSIBLE. (X)
