NTSB accidents & incidents
Operated by EVERTS AIR FUEL INC (per NTSB report)
An in-flight collision with birds during takeoff.
Operated by Everts Air Fuel Inc (per NTSB report)
The pilot's failure to attain the proper touchdown speed and his decision to land with a tailwind without ensuring that there was adequate runway length for the touchdown.
Operated by EVERTS AIR FUEL, INC. (per NTSB report)
The pilot's inadequate preflight fuel planning, his reliance on the fuel flow indicator without verifying the initial fuel level input, and his improper decision to continue the flight with the low fuel light illuminated, which resulted in fuel exhaustion, the subsequent total loss of engine power, and a forced landing on steep, rocky terrain.
Operated by EVERTS AIR FUEL INC (per NTSB report)
The failure of the right main landing gear side brace due to fatigue, which resulted in a collapse of the right main landing gear.

Maintenance disclosure history
Service Difficulty Reports are self-reported maintenance findings. The NUMBER of reports is not a measure of reliability — diligent operators file more, not fewer. These are shown for transparency, not as a negative signal. To verify a report, search its control number at the FAA SDRS site.
Reports span 2001–2013. Most-reported systems: Landing gear (32) — 2; Exhaust (78) — 2; Air conditioning & press. (21) — 1; Flight controls (27) — 1; Propellers (61) — 1 (top 5 of 6 systems, covering 7 of 8 coded reports). Most-common natures of condition, as filed: OTHER — 3; WARNING INDICATION — 3; FLAME/FIRE; WARNING INDICATION — 1; FLT CONT AFFECTED — 1.
8 reports on file.
Part: LANDING GEAR — COLLAPSED (RIGHT) · airframe 35,183 hrs
RT MLG COLLAPSED ON PARKING CAUSING DAMAGE TO PROP AND RIGHT WING.
Part: EXHAUST PIPE — RUPTURED (NR 2 ENGINE) · airframe 32,290 hrs
NR2 ENGINE FIRE WARNING ON TAKEOFF (ZONE 2 & 3). REPLACED NR 2 ENGINE IAW MM. INSPECTION OF REMOVED ENGINE REVEALED NO FIRE DAMAGE BUT OVERHEAT DAMAGE CAUSED BY A RUPTURED EXHAUST JOINT ON THE NR 9 CYLINDER.
Part: TRIM TAB — BINDING (AILERONS) · airframe 24,287 hrs
AILERONS BINDING ON TAKEOFF AFTER GEAR RETRACTION (BEFORE TAKEOFF CONTROL CHECK) NORMAL. FOUND THAT THE AILERON SPRING TAB MECHANISM WAS BINDING PREVENTING MOVEMENT OF THE TAB SYSTEM WHICH IN TURN INCREASED THE FORCES REQUIRED TO MOVE THE AILERONS.
Part: WHEEL — SHEARED (MLG)
DRIVE KEY SCREWS SHEARED AND SEVERAL BACKED OUT. KEY LUGS SEVERELY DAMAGED.
Part: CLAMP — OUT OF POSITION (EXHAUST)
EXHAUST COLLECTOR CONNECTION MOVED OUT OF POSITION AFTER TAKEOFF, ALLOWING EXHAUST GAS TO ESCAPE AND ILLUMINATE THE ZONE 1 FIRE WARNING. ENGINE WAS SHUTDOWN AND AIRCRAFT RETURNED TO DEPARTURE AIRPORT. AIRCRAFT RECEIVED NO DAMAGE AND REPAIRS WERE ACCOMPLISHED. (K)
Part: MOTOR — SHORTED (HEATER BLOWER)
AFTER COMPLETION OF HOLD ITEMS COCKPIT FILLED WITH LIGHT SMOKE ACCOMPANIED BY ODOR OF ELECTRICAL FIRE. THE SOURCE OF THE SMOKE WAS FOUND TO BE THE COCKPIT HEATER BLOWER MOTOR WHICH WAS SUBSEQUENTLY REPLACED.
Part: INTAKE PIPE — CRACKED (NR 10 CYLINDER)
EXPERIENCED AN ENGINE FIRE ACCOMPANIED BY BOTH AUDIO AND VISUAL WARNING. FIRE WAS OBSERVED FROM THE COWLING. WHEN THE THROTTLE WAS PULLED BACK THE FIRE WENT OUT AND ALL WARNINGS STOPPED. THE ENGINE WAS SHUT DOWN AND THE AIRCRAFT DIVERTED. SUBSEQUENT INVESTIGATION FOUND THAT THE INTAKE WYE PIPE TO NR 10 CYLINDER HAD SPLIT ALLOWING RAW FUEL/ AIR MIXTURE TO BE BLOWN DIRECTLY ON TO A HOT EXHAUST PIPE. REPLACEMENT OF THE PIPE AND SEALS FIXED THE PROBLEM.
Part: GOVERNOR — FAULTY (PROPELLER)
NR 1 ENGINE FEATHERED DUE TO UNCONTROLABLE RPM CAUSED BY A FAULTY PROP GOVERNOR. REPLACED GOVERNOR, GROUND CHECK, GOOD.
