FLIGHT WIS804 FROM KMKE (MILWAUKEE) TO KGRB (GREEN BAY). ON APPROACH PILOT EXTENDED LANDING GEAR FOR LANDING AND DID NOT GET A NOSE GEAR GREEN LIGHT INDICATION. CYCLED GEAR SEVERAL TIMES INCLUDING MANUAL EXTENSION PER THE AFM EMERGENCY PROCEDURES BUT STILL DID NOT GET THE GREEN LIGHT INDICATION AND WHEN REDUCING THE POWER GOT THE GEAR WARNING HORN. WIS804 WAS DIVERTED TO KOSH (OSHKOSH) PER COMPANYâS INSTRUCTION. WIS804 FLEW BY TOWER AND COMPANY PERSONAL ON THE GROUND AND WAS DETERMINED THAT ALL 3 GEAR WERE DOWN BUT STILL DID NOT HAVE THE NOSE GEAR GREEN LIGHT. DECISION WAS MADE TO LAND THE AIRCRAFT ON RUNWAY 9 AND EMERGENCY SERVICES WERE DISPATCHED BY THE TOWER AS PRECAUTION. WIS804 EXPERIENCED A HARD LANDING AND THE LEFT MAIN GEAR COLLAPSED AND AIRCRAFT CAME TO REST ON THE RUNWAY.
PILOT DEPARTED KANE, WHILE CLIMBING THROUGH 2,700', LEFT ENGINE DEVELOPED A VIBRATION. WHILE SCANNING ENGINE INSTRUMENTS HE NOTICED THE LEFT ENGINE OIL PRESSURE WAS AT ZERO PSI. PILOT TURNED AROUND TO RETURN TO KANE AIRPORT AND BEGAN ENGINE SHUT DOWN PROCEDURES AND TO FEATHER ENGINE. OIL PRESSURE RETURNED BRIEFLY BUT AGAIN FELL TO ZERO. PILOT NOTICED ORANGE FLAMES COMING FROM TOP FRONT OF COWLING. PILOT DECLARED AN EMERGENCY AND LANDED AIRCRAFT. HE REPORTED THE FLAMES DIED DOWN AND SELF EXTINGUISHED ON ROLL OUT. INITIAL INSPECTION OF THE ENGINE SHOWED THE #6 CONNECTING ROD HAD BROKE AND PUNCHED THROUGH THE CASE AND THE METAL FROM THE CASE HAD SEVERED THE FITTING ON THE FUEL MANIFOLD ON TOP OF THE ENGINE ALLOWING RAW FUEL TO DISCHARGE INTO THE ENGINE COMPARTMENT UNTIL THE PILOT TURNED THE FUEL SELECTOR TO THE OFF POSITION.
VACUUM PUMP FAILED IN LESS THAN 10 HOURS OPERATION. PROBABLY CAUSED BY DE-ICE CONTROL. VALVE RELIEF PRESSURE SET AT 5 PSI, SHOULD BE SET AT 1.5 PSI TO PREVENT OVERWORKING VACUUM PUMP.
PILOT PLACED GEAR HANDLE IN THE DOWN POSITION, LOUD BANG, ALL 3 GEAR DOWN LIGHTS ILLUMINATED AND PILOT LANDED WITHOUT ISSUE. AFTER SHUT DOWN NOTICED LINKAGE HANGING FROM NOSE WHEEL WELL. THE NOSE GEAR TORQUE TUBE ADJUST INBD P/N 0842104-3 HAD FAILED AND THE FORK BOLT P/N 5243518-2 WAS SLIGHTLY BENT.
RIGHT MAIN LANDING GEAR COLLAPSED AFTER LANDING. PILOT MADE A LEFT TURN CAUSING RIGHT GEAR TO COLLAPSE. INSPECTION FOUND THE TORQUE TUBE BRACKET ASSEMBLY BROKEN AT THE PIVOT BEARING, CAUSING THE DOWN LOCK TENSION TO BE NONEXISTENT. FAILURE OF THE TORQUE RUBE ASSEMBLY DID NOT APPLY SUFFICIENT DOWN LOCK TENSION.
AFTER TAKE OFF AND IN CLIMB OUT, THE TOWER REPORTED SMOKE TRAILING THE LEFT ENGINE. RETURNED TO DEPARTURE WITHOUT INCIDENT. REMOVED COWL AND FOUND OIL DRIPPING FROM THE NR 3 CYLINDER. FOUND THE GASKET ON THE NR 3 CYLINDER HAD COLLAPSED IN UPPER RIGHT CORNER. REPLACED THE VALVE COVER GASKET AND CLEANED OIL RESIDUE. RAN ENGINE FOR LEAK AND OPERATIONAL CHECKS, NO LEAKS AND PERFORMANCE NORMAL.
DURING DESCENT INTO KANE AIRPORT PILOT COULD SEE OIL STREAKING BACK FROM TOP COWL LOUVERS. AFTER A FEW MINUTES HE ALSO NOTICED OIL PRESSURE DROPPING. PILOT PRECAUTIONARY SHUT DOWN ENGINE AND FEATHERED PROPELLER. SINCE KANE AIRPORT WAS THE CLOSEST AIRPORT AND ALSO HIS DESTINATION CONTINUED DESCENT AND LANDED WITHOUT ISSUE. MAINTENANCE REMOVED COWLING AND FOUND OIL AFT OF THE ACCESSORY GEARBOX AND APPEARED TO BE COMING FROM AROUND THE TURBO AREA. UPON FURTHER INVESTIGATION FOUND OIL BYPASSING THE SEALS ON THE TURBO.
RIGHT ENGINE STARTED RUNNING A BIT ROUGH. VISUALLY INSPECTED MAGNETOS AND REMOVED RIGHT MAGNETO FOR INSPECTION. DISCOVERED WORN DISTRIBUTOR GEAR. REPLACED MAGNETO WITH OVERHAULED UNIT OF SAME TYPE.
AIRCRAFT WAS IN ROUTE TO GRB FROM LAN. SHORTLY AFTER CLIMB OUT AND AIRCRAFT WAS LEVEL IN CRUISE AT 16,000FT PILOT HEARD A LOUD POP AND CABIN DEPRESSURIZED. PILOT PERFORMED EMERGENCY DESCENT AND DIVERTED TO GRR AND PASSENGERS WERE PICKED UP AND FLOWN TO THEIR DESTINATION. MAINTENANCE INSPECTED THE CABIN PRESSURIZATION SYSTEM AND FOUND A FLEXIBLE DUCT HAS COME LOOSE AND SEPARATED FROM ONE END OF THE SUPPLY DUCT. MAINTENANCE INSPECTED THE DUCT, FLEXIBLE DUCT AND CLAMP AND FOUND ALL THREE TO BE SERVICEABLE. MAINTENANCE RESECURED THE FLEXIBLE DUCT TO SUPPLY DUCT AND TIGHTENED THE CLAMP AS REQUIRED. INSPECTED REMAINING CLAMPS FOR TIGHTNESS AND SECURITY. TESTED THE PRESSURIZATION SYSTEM AND CHECKED NORMAL. RELEASED AIRCRAFT RETURNED THE BASE.
