N499GS TRIP 8907, 2 OCT 2025, LEG 1 KBNA KMCN. NO PAX ONBOARD. DURING CRUISE AT FL450 THE CABIN BEGAN DEPRESSURIZING. RECEIVED CABIN ALTITUDE AMBER, THEN MASTER WARNING (WITHIN AN APPROXIMATELY ONE MINUTE OF EACH OTHER) AND PERFORMED MEMORY ITEMS FOR THE EMERGENCY DESCENT. I DECLARED AN EMERGENCY, RAN CHECKLISTS, AND DIVERTED TO A NEARBY SUITABLE AIRPORT (KMCN) WHERE THE WEATHER WAS BETTER THAN THE PLANNED DESTINATION AND THE AIRPORT IS TOWERED WITH LONGER RUNWAYS AND MORE SERVICES
IN THE AFTERNOON (IN LIML) FOLLOWING CHECKS ON THE FIRE-FIGHTING SYSTEM THAT WERE BEING CARRIED OUT BY THE PERSONNEL IN CHARGE, THE FIRE-FIGHTING SYSTEM IN HANGAR 6 WAS ACTIVATED. IN THE HANGAR UNDER MAINTENANCE THERE WAS AIRCRAFT REGISTRATION MARKS N445TG (LEARJET) WHICH WAS AFFECTED MARGINALLY BY THE SPILLAGE OF THE FOAM. THE AIRCRAFT AND ENGINE MANUFACTURERS HAVE BEEN INFORMED AND INVOLVED FOR APPROPRIATE ACTIONS. THE OPERATOR WAS PROMPTLY NOTIFIED.
LH T/R HAS BEEN PLACED ON MEL AND WAITING ON A SWITCH/WIRE HARNESS. THE MANUFACTURE HAS A BACK ORDER LOG ON THE SWITCH AND CAN NOT GIVE AN UPDATE ON WHEN A REPLACEMENT CAN BE MANUFACTURED
EMERGENCY BATTERY OVERHEAT INDICATION IN FLIGHT.
DURING A POSITIONING FLIGHT FROM KPNE TO KHPN, THE CREW EXPERIENCED AN ABNORMAL OIL INDICATION ON ENGINE NO. 1. THE CHECKLIST DIRECTED THE CREW TO SHUTDOWN THE ENGINE AS A PRECAUTIONARY ACTION. THE CREW ASSESSED THE PROBLEM, DECLARED AN EMERGENCY AND LANDED SAFELY IN KHPN. AFTER MAINTENANCE WAS DISPATCHED TO THE AIRCRAFT AN INVESTIGATION OF THE NO. 1 ENGINE FOUND THAT THE ALTERNATOR HAS SHEARED ITS SHAFT AND THAT THE OIL VACATED THE ENGINE THROUGH THAT OPENING. A FADEC READING HAS BEEN DIRECTED TO BE ACCOMPLISHED AND THE FINDINGS WILL DETERMINE WHAT ACTION WILL BE TAKEN NEXT.
SMOKE IN THE CABIN DURING CLIMB. CREW DECLARED EMERGENCY AND RETURNED TO DEPARTING AIRPORT KTMB. MECHANIC NOTICED OIL ACCUMULATING ON RH ENGINE INLET. PRATT & WHITNEY IN PARTNERSHIP WITH WEST STAR TROUBLESHOT TO ENGINE OIL OVER-SERVICE. MECHANICAL DIFFICULTY REPORT SUBMITTED TO CHDO.
SMOKE IN THE CABIN DURING CLIMB. CREW DECLARED EMERGENCY AND RETURNED TO DEPARTING AIRPORT KTMB. MECHANIC NOTICED OIL ACCUMULATING ON RH ENGINE INLET. PRATT & WHITNEY IN PARTNERSHIP WITH WEST STAR TROUBLESHOT TO ENGINE OIL OVER-SERVICE. MECHANICAL DIFFICULTY REPORT SUBMITTED TO CHDO.
DOOR SEAL LIGHT CAME ON AT FLT LEVEL 410. DOOR SEAL SQUEALED THEN LOST PSI ON DECENT. DECLARED AN EMERGENCY AND LANDED AT KPHL.
DEPARTED, DURING CLIMBOUT THROUGH 16,000 FT RECEIVED A LEFT LOW FUEL PRESSURE LIGHT FOLLOWED BY STANDBY BOOST PUMP LIGHT. THE CREW COMPLETED THE CHECK LIST AND BOTH LIGHTS REMAINED ON.
AFTER A NORMAL TAKEOFF, THE AUTO-THROTTLES ATTEMPTED TO REDUCE THE POWER FOR A CLIMB POWER SETTING. THE LT ENGINE RESPONDED ACCORDINGLY; THE RT ENGINE, HOWEVER, REMAINED AT FULL TAKEOFF POWER.
45 MINUTES INTO THE FLIGHT, BEGAN TO NOTICE AN OIL SMELL IN THE CABIN WITH NO OTHER ABNORMAL INDICATIONS. UPON COMMENCING OUR INITIAL DECENT, BEGAN TO HAVE MODERATE ENVIRONMENTAL SMOKE IN THE CABIN & THE BLEED OVERHEAT LIGHT CAME ON AGAIN. WE COMPLETED THE SMOKE IN THE CABIN.
APPROXIMATELY TEN MINUTES TO LANDING, COCKPIT COLD AIR VENTS & DIFFUSERS STARTED TO BLOW VERY HOT AIR & FAINT SMOKE WAS OBSERVED FILLING THE COCKPIT.
NOSE LANDING WOULD NOT RETRACT WHEN SELECTED.
CABIN ALTITUDE TOD CABIN ALTITUDE INCREASED VIELENTY AT 200 FT + IAE MINUTE ALL THE WAY TO 20,000 FT. PAX O2 MASK PARTIALLY DEPLOYED. DOOR SEAL SHIFTED.
HAD TO RETURN TO THE AIRPORT BECAUSE THERE WAS 30 TO 90 DEGREES HEADING DIFFERENCE DUE TO A MISS ALIGNMENT OF THE IRS SYSTEM.
CREW REPORTS RT OTBD TIRE TREAD SEPARATED.
NUMBER 3 THROTTLE WAS STUCK IN THE IDLE POSITION. FURTHER INVESTIGATION FOUND FOUR MOUNTING SCREWS FOR BLEED AIR ANTI-ICE SWIVEL WERE LOOSE. THERE WAS THERMAL DAMAGE TO UPPER COWL DOOR, BURNT & DELAMINATION. TELEFLEX CABLE MELTED INTERNALLY, ENGINE AFTER BODY THERMAL DAMAGE.
CREW REPORTED LT MAIN FUEL BOOST PUMP FAILED IN FLIGHT.
AT ABOUT 1200 FT ON CLIMB OUT, CREW NOTICED AURAL WARNING & THE CAS SHOWED A RED L REV UNLOCKED INDICATION.
APPROXIMATELY 1 HOUR INTO CRUISE FLIGHT, WHILE STABLE AT FL 400, A LOUD POP WAS HEARD FROM REAR OF ACFT. NOTICED NR 3 ENGINE FUEL COMPUTER LIGHT ILLUMINATED ON ANNUNCIATOR PANEL AND HEARD THE ENGINE SPOOLING DOWN. OBSERVED NO OIL PRESSURE ON ENGINE, SECURED ENGINE. DECLARED EMERGENCY. RAN ONE ENGINE INOPERIVE CHECKLIST. LANDED WITHOUT INCIDENT.
GRADUAL DECREASE IN POWER TO RIGHT ENGINE. FOUND BLEED BAND DEBONDED.
AFTER LEVEL OFF AT FL380 AND APPROX 18 MINUTES INTO THE FLIGHT THE "ENG 1 FUEL" CAUTION LIGHT ON THE OVERHEAD PANEL ILLUMINATED, INDICATING "EITHER FUEL FILTER BLOCKED OR FUEL OVERHEAT" IAW THE AIRPLANE FLIGHT MANUAL THE NR 1 ENGINE THRUST LEVER WAS REDUCED TO IDLE AND CLEARANCE FOR A LOWER ALTITUDE AND RETURN TO DEPARTURE WAS REQUESTED. NO OTHER ENGINE MALFUNCTIONS WERE INDICATED WITH N1, N2, ITT, OIL PRESSURE AND FUEL FLOW ALL STABLE. THE LIGHT CANCELLED DURING THE TURN AND DESCENT AND THE FLIGHT CONTINUED TO LANDING WITH NO FURTHER INDICATIONS OF MALFUNCTION.
INVESTIGATED PILOT REPORT OF LT FUEL BOOST PUMP INDICATION. FOUND LT FUEL BOOST PUMP INOPERATIVE. INSTALLED BOOST PUMP WAS O/H 2/04/2011. REPLACED LT BOOST PUMP WITH AN O/H, PN 2C40-2, BOOST PUMP AS ORIGINALLY INSTALLED, OPS CKS OK. PUMP FAILURE MODE UNKNOWN, RECOMMEND INVESTIGATION AT TEAR-DOWN BY O/H FACILITY. PN 2070C.11 BOOST PUMPS WERE REPLACED BY OTHER PN 2C40-2 BOOST PUMPS YEARS PREVIOUSLY AT MFG AND RETROFITTED TO EARLIER ACFT SB 28-88 AND MODIFICATION 253256A. THE ACFT PARTS DISTRIBUTION DIVISION SHOW PN 2070C.11 PUMPS AS INTERCHANGEABLE WITH 2C40-2 BOOST PUMPS.
FLIGHT CREW REPORTED AN UNLOCKED LANDING GEAR/DOOR INDICATION FOLLOWING GEAR RETRACTION AFTER DEPARTURE. LANDING GEAR WAS CYCLED AND INDICATIONS WERE NORMAL FOLLOWING RETRACTION. NO UNUSUAL NOISE/BUFFETING, NOTED DURING CRUISE FLIGHT. UPON ARRIVAL NOTED THE LT WHEEL WELL (INBD) DOOR SAGGING OPEN AT THE AFT END. FOUND THE BRACKET (800XP IPC 32-10-00-10 NR 215) ATTACHING THE WHEEL WELL DOOR AFT SUPPORT ASSY (800XP IPC 32-10-00-10 NR 185) CRACKED ACROSS THE MOUNTING LUGS FOR THE PN 25FC5805 AFT SUPPORT ASSY. ATTACH BOLT (800XP IPC 32-10-00-10 NO. 180). 1 (OF 4) MOUNTING LUG SHOWS POSSIBLE EVIDENCE OF PRE-EXISTING CRACK APPROX HALFWAY THROUGH LUG. FAILURE MODE UNKOWN, POSSIBLY CAUSED BY DOOR MISRIGGING OR FATIGUE. RECOMMEND MFG INVESTIGATE WHETHER A RELATIVELY ISOLATED INCIDENT OR IF MORE FREQUENT OR MORE DETAILED INSPECTIONS ARE WARRANTED.
CO-PILOTS WINDSHIELD CRACKED IN FLIGHT AT CRUISE, DUE TO WINDSHIELD INTERNAL HEATER TERMINAL SHORT.
