#2 ENGINE FIRE INDICATION OBSERVED DURING CLIMB OUT FROM KBED. CREW FOLLOWED QRH PROCEDURES AS REQUIRED. ENGINE FIRE INDICATION FOR #2 ENGINE ON CLIMB OUT. FOLLOWED QRH PROCEDURES. USED BOTH EXTINGUISHERS. INDICATION CONTINUED. SECURED ENGINE AND LANDED SINGLE ENGINE. INDICATION WAS FALSE. THERE WAS NO ENGINE FIRE.
DISCOVERED DURING GROUND OPERATION. NO AIR WAS FELT AT THE BACK OF THE TAT PROBES. INSPECTION OF THE BLEED AIR FEED LINE STARTING AT THE TAT SOLENOID IN THE AFT EQUIPMENT BAY AREA FOUND A LINE HAD SEPARATED JUST AFTER AN ADEL SUPPORT CLAMP. NO ADDITIONAL SURROUNDING DAMAGE WAS FOUND DUE TO THIS BREAK. GULFSTREAM FAST WAS CALLED AND THEY PERMASWAGED A NEW LINE IN PLACE. OPERATIONAL GROUND CHECK OF TAT PROBE WAS SATISFACTORY WITH AIR EXISTING THE AFT END OF BOTH LEFT AND RIGHT PROBES.

THE CUSTOMER REPORTED THAT THE CREW OXYGEN SYSTEM LOST PRESSURE OVER A FEW DAYS. DURING TROUBLESHOOTING OF THE SYSTEM THE TECHNICIAN FOUND THE ADAPTER FITTING, P/N 500189, LOCATED BETWEEN THE TUBING NUT AND THE PRESSURE GAUGE, P/N 171013-00, FOR THE CREW OXYGEN BOTTLE, LOCATED IN EXTERNAL OXYGEN SERVICING PANEL. FURTHER INSPECTION REVEALED THAT THE FITTING WAS CRACKED LONGITUDINALLY ON THE FEMALE PORTION OF THE FITTING, WHICH IS A PIPE THREAD. THIS CRACK IS EXACTLY THE SAME AS PREVIOUSLY REPORTED AND PRINTED IN AC 43-16A. A NEW GUAGE, P/N 171013-00 WHICH INCLUDES THE FITTING, P/N 500189, WAS INSTALLED AND LEAK CHECKED. NO LEAKS WERE NOTED.

RT THRUST REVERSER ILLUMINTED IN FLIGHT, SYSTEM OPERATED CORRECTLY IWA MFG MM. VISUAL INSPECTION REVEALED STICKY LATCHES AND DEFECTIVE T/R ACCUMULATOR. AWAITING FINAL FIX AND RETURN TO SERVICE. REMOVED AND REPLACED ACCUMUALTOR AND INDICATOR GAUGE. LATCHES WERE CLEANED AND LUBRICATED. AS PRECAUTION, T/R SELECTOR VALVES WERE REPLACED ALONG WITH THE SYSTEM RELIEF VALVE AND THE RT SIDE T/R ACTUATOR. ALL CHECKS, NORMAL.
DURING CLIMB, LANDING GEAR DID NOT RETRACT WHEN SELECT UP ON FIRST RETRACTION ATTEMPT. SELECTOR HANDLE CYCLED GEAR OPERATED NORMALLY ON ALL SUBSEQUENT SELECTIONS. ACCOMPLISHED OPERATION CHECKS COULD NOT DUPLICATE, TROUBLESHOT SYSTEM AND REPLACED SUSPECTED INTERMITTENT MAIN LANDING GEAR DOOR CONTROL VALVE. ACCOMPLISHED NUMEROUS GEAR SWINGS TO INSURE PROPER OPERATION AND AIRCRAFT WAS RETURNED TO SERVICE. (M)

UPON POST FLIGHT INSPECTION, CREW NOTICED OIL LEAKING FROM THE NR 3 ENGINE COWLING AND OIL FILTER BYPASS BUTTON WAS POPPED. AIRFRAME TT 9153.0 HOURS. NR 3 ENGINE TT 8958.2 HOURS NR3 ENGINE CYCLES 6436. AIRCRAFT WAS INSPECTED BY MAINTENANCE AND SUSPECTED AN INTERNAL FAILURE. LOANER ENGINE WAS INSTALLED IN THE NR 3 POSITION. ORIGINAL NR 3 ENGINE WAS SENT TO MFG FOR TEARDOWN INSPECTION.
ON TAKEOFF CLIMB, LANDING GEAR FAILED TO RETRACT, TRANSPONDER DID NOT REPORT, CABIN FAILED TO PRESSURIZE. INSPECTED AND FOUND DEFECTIVE LEFT MAIN GEAR SQUAT SWITCH. REMOVED LEFT SQUAT SWITCH AND INSTALLED A NEW UNIT IAW LEAR 60 MM. OPERATIONAL CHECK, GOOD. UNEVENTFUL PRECAUTIONARY LANDING.
UPON LANDING GEAR EXTENSION, DURING FINAL APPROACH, RIGHT MAIN GEAR GREEN DOWN AND LOCK LIGHT DID NOT ILLUMINATE. PERFORMED LIGHT TEST AND CONFIRMED LIGHT INOPERATIVE. REPLACED BULBS IN LANDING GEAR CONTROL PANEL. JACKED AIRCRAFT AND PERFORMED LANDING GEAR FUNCTIONAL CHECK. OPS CHECK GOOD IAW LEARJET MM. FLT CREWS SUSPECTED GEAR DOWN LIGHT INDICATION BULB TO BE AT FAULT. RAN ALL APPROPRIATE CHECKS AND AS FURTHER PRECAUTION REQUESTED TOWER FLYBY FOR LANDING GEAR DOWN VERIFICATION. (M)
AIRCRAFT WAS IN CRUISE AT FL 35000, WHEN CREW NOTICED THAT THE AIRCRAFT BEGAN TO GRADUALLY DEPRESSURIZE. THE CREW ELECTED TO DESCEND TO A LOWER ALTITUDE. DURING DESCENT CABIN PRESSURE DROP CAUSED THE AURAL WARNING HORN TO SOUND AND PAX OXYGEN MASKS TO DEPLOY. CREW THEN DECLARED AN EMERGENCY AND DIVERTED TO BUF INTERNATIONAL AIRCRAFT MADE NORMAL LANDING. AIRCRAFT WAS INSPECTED AND FOUND CLAMPS SECURING MAIN CABIN AIR SUPPLY TUBE FLEX COUPLINGS HAD LOOSENED CAUSING LEAKAGE AND SUBSEQUENT LOSS OF PRESSURIZATION SYSTEM EFFICIENCY. ALL COUPLINGS CLAMPS SECURED A ACCOMPLISHED SYSTEM TEST. CHECKED OK.
PROPELLER HAS NEW STYLE BLADE LOCKS. COTTERPIN THAT HOLDS SPRING AND LOCK IN ASSEMBLY IS FAILING OR WEARING OUT THE HOLES IN HOUSING.

THE FIRE WARNING SYSTEM TESTS NORMAL. A & P MECHANIC AT GSP REMOVED RT ENGINE COWLS. CHECKED AND CLEANED FIRE LOOP CONNECTORS. PERFORMED VISUAL INSPECTION OF ENGINE AND FOUND NO EVIDENCE OF ENGINE FIRE OR BLEED AIR LEAKS. SECURED ENGINE COWLS. AIRCRAFT RETURNED TO SERVICE.

DURING APPROACH TO TOL, EXTENDED LANDING GEAR AND NOSE GEAR, UNSAFE LIGHT REMAINED ILLUMINATED. CYCLED LANDING GEAR, NORMAL DOWN AND LOCKED INDICATION. CONTACTED TOL TOWER TO CONFIRM NOSE GEAR DOWN. AIRCRAFT LANDED WITHOUT INCIDENT. NOSE GEAR STRUT DEFLATED AND LEAKING HYDRAULIC FLUID. REPLACED NOSE STRUT SEALS, SERVICED STRUT. AIRCRAFT RETURNED TO SERVICE.

ISP - AFTER TAKEOFF, CABIN AND COCKPIT FILLED WITH SMOKE (ENGINE OIL FUMES). EMERGENCY ACTIONS WERE TAKEN AND AIRCRAFT RETURNED TO ISP WITHOUT INCIDENT. BORESCOPE INSPECTION OF ENGINE COMPRESSOR SECTION REVEALED OIL PRESENT IN THE NR 4 AND NR 5 BEARING CARBON SEAL CAVITY. OIL LEAKAGE PAST THE NR 4 AND NR 5 AIR/OIL SEALS RESULTED IN SMOKE IN THE CABIN. BOTH THE NR 4 AND NR 5 SEALS WERE REPLACED AND THE NR 5 SEAL AREA SCAVENGE CAPABILITY IMPROVED WITH THE INCORPORATION OF SB 72-3540. THE ENGINE HAS BEEN REASSEMBLED AND HAS DEMONSTRATED ACCEPTABLE PERFORMANCE/FUNCTION ON TEST CELL AND RETURNED TO SERVICE.
ON 1-11-95 DURING APPROACH TO ISP, LT ENGINE CHIP DETECTOR WARNING LIGHT ILLUMINATED. AIRCRAFT LANDED WTIHOUT FURTHER INCIDENT. INSPECTION OF CHIP DETECTOR REVEALED A PIECE OF METAL BRIDGED ACROSS CHIP DETECTOR CONTACTS. ENGINE REMOVED FROM SERVICE. TEAR DOWN REPORT WILL FOLLOW.
