AT 30 MIN CRUISE FLIGHT @ FL400, CAPTAIN (PF) NOTICED LT SIDE PFD IAS FLAG, AND VERBALIZED TO PNF. PF COMPARED PFDS AND STBY ASI. RT SIDE PFD AND STBY ASIS INDICATED APPRX 20KTS FASTER (220KTS) THAN LT SIDE. OVER THE COURSE OF 30SEC, AIRSPEED CONTINUED TO DECAY BELOW 200KTS ON LT SIDE PFD AND DECREASED AT SAME RATE ON BOTH RT SIDE PFD AND STBY. PF DEEMED A DESCENT NECESSARY TO TRADE ALTITUDE FOR AIRSPEED. PNF ASKED ATC FOR LOWER ALTITUDE. PF INITIATED A ~3000FPM DESCENT AND AIRSPEED CONTINUED TO DECAY TOWARD 140KTS ON ALL ASIS. PF QUICKLY REALIZED ALL AIRSPEED INFORMATION WAS ERRONEOUS AND SUSPECTED STUCK PITOT. BOTH PILOTS VERIFIED PITOT HEATS-ON AND CIRCUIT BREAKERS-IN. PF DECREASED DESCENT TO 1000FPM, SET A KNOWN POWER SETTING AND REFERENCED AOA .2. IAS PEGGED AT 40KTS ON BOTH PFDS AND 0KTS ON STBY. PNF DECLARED EMERGENCY AND ASKED FOR VECTORS TO KSLN. PNF RAN LOSS OF AIRSPEED INDICATION CHECKLIST AND REVIEWED LOSS OF AIRSPEED APPROACH CHECKLIST. ALL ASIS RETURNED TO NORMAL @ ~FL260 AND INDICATED SIMILAR AIRSPEEDS. PF FLEW ILS UNEVENTFULLY INTO KSLN.
TCU SYSTEM FAILED DURING TAKE-OFF / CLIMB.
PILOT STATED LOSS OF PRESSURIZATION AND DECREASED MANIFOLD PRESSURE. EXHAUST PIPE BROKEN.
RIGHT MLG TIRE TREAD SEPARATED, BUT DID NOT GO FLAT. TREAD WHIPPED AROUND AND PUNCTURED THE FLAP, DAMAGED THE FAIRING BETWEEN THE REAR SPAR AND FLAP LEADING EDGE, BENT THE MAIN GEAR DOOR ATTACH ROD, BROKE A BONDING BRAID, DAMAGED THE WIRE BUNDLE PROTECTIVE COATING THAT COVERS THE WIRES FOR THE ANTI-SKID GENERATOR, AND POSSIBLY CAUSED DELAMINATION OF THE GEAR DOOR.
OIL PRESSURE WARNING ANNUNCIATION, LOSS OF OIL PRESSURE. FOUND BREATHER TUBE GARLOCK SEAL TO BE ALLOWING OIL TO BE BLOWN OUT OF THE ENGINE.
LOST MAIN HYD SYS PRESSURE INDICATION. DECLARED AN EMERGENCY LANDING.

LT STARTER WOULD NOT ENGAGE.
(UVVR) WHILE AIRCRAFT WAS STRIPPED FOR A PAINT JOB, VISUAL INSPECTION OF LT WING OVERWING FAIRING WAS DELAMANATED. AFTER FUTHER PANEL REMOVAL, FOUND CORROSION EXTENDING UNDER OVERWING FAIRING. CORROSION EXTENDING THROUGH LT WING PLANK AT W.S. 102.5 TO W.S. 107 APPROX. 10 INCHES FROM FRONT SPAR EXTENDING APPROX. 15 INCHES FROM FRONT SPAR (CORROSION FOUND IS EXFOLIATED). THIS AREA IS ONLY INSPECTED BY VISUAL MEANS DURING A 150 HOUR INSPECTION AND THE WING PLANKS ARE COVERED UP BY THE OVERWING FAIRINGS WHICH ARE FIBERGLASS OVERLAYS

RT ENGINE WOULD NOT START. EXCITER INOPERATIVE.
ON LANDING THE ENG POWER HUNG AT APP 800 LBS OF TORQUE. FOUND FUEL CONTROL UNIT FAULTY.
RT MAIN GEAR, DOWN AND LOCKED GREEN INDICATOR LIGHT, WOULD NOT EXTINGUISH.
LT MLG BEGAN LOWERING OUT OF THE NACELLE DURING FLIGHT.
AT CRUISE FLIGHT, AFTER 1 HR 27 MIN INTO FLIGHT, THE LT OIL PRESSURE INDICATOR FLUCTUATED FROM APPROXIMATELY 42-45 PSI DOWN TO 30-35 PSI TWICE PER SECOND CONTINUOUSLY. OIL TEMP REMAINED NORMAL, ALL OTHER ENGINE INDICATIONS NORMAL, (N1, ITT, N2). APPROXIMATELY 4-6 MINUTES LATER THE OIL PRESSURE INDICATOR FLUCTUATED DOWN TO 25-35 PSI, THE LT ENGINE "OIL PRESS LOW" ANNUCIATOR ILLUMINATED. THE SIC REDUCED POWER TO IDLE, EXECUTED SHUTDOWN PROCEDURES IAW CHECKLIST PROCEDURE. THE PIC DECLARED AN EMERGENCY AND DIVERTED . THE LT ENGINE OIL FILTER BYPASS INDICATOR WAS FOUND POPPED, METAL PIECES WERE FOUND IN THE FILTER AND TRANSFER GEARBOX, AND THE NR 6 BEARING, OIL PRESSURE LINE WAS FOUND CRACKED. (K)
ON AUGUST 16, 2008, AT APPROXIMATELY 8:30 AM, AFTER LANDING, THE CREW HEARD AUDIBLE WARNING OF HYDRAULIC SYSTEM LOW PRESSURE. THE AC TAXIED TO THE RAMP OF DESTINATION, WITHOUT INCIDENT. UPON INVESTIGATION, THE HYDRAULIC PUMP WAS FOUND TO HAVE FAILED. (K)

TWELVE BROKEN RIVETS IN A ROW RESULTING IN THE SEPARATION OF RIGHT HAND FRAME STATION 298.562 P/N 50-440014-305 AND ANGLE P/N 50-440014-307 JUST AFT OF THE RIGHT SIDE ESCAPE HATCH OPENING.

LT ENGINE VIBRATION NOTICED, UPON INVESTIGATION, FOUND THE STARTER/GENERATOR BEARINGS GOING BAD.
WHEN DEPARTING FROM SPIRIT OF ST. LOUIS AIRPORT S.U.S., THE CREW RETRACTED THE MAIN GEAR, UPON COMPLETION OF THE UP CYCLE THE CREW HEARD A KNOCKING NOISE FROM THE BELLY OF THE AIRCRAFT, AND THEN GOT A RED LIGHT IN THE GEAR HANDLE. THEY PUT THE GEAR DOWN AND HAD GOOD INDICATION. THEN PERFORMED A FLY-BY AND TOWER SAID ALL LOOKED NORMAL. THE AIRCRAFT LANDED WITHOUT INCIDENT.
AFTER DEPARTURE, PLTS NOTICED RT ENG OIL PRESSURE WAS ABOVE THE GREEN ARC INTO THE YELLOW. PULLED BACK ON THE THROTTLE AND THE OIL PRESSURE FOLLOWED THE THROTTLE MOVEMENT, AT THAT TIME THE ENGINE BEGAN TO SURGE AND THEY HEARD A POP FROM THE REAR OF THE A/C. THEN THE ENGINE BEGAN TO SPOOL DOWN. CREW BEGAN THE S/D PROCEDURE AND RETURNED TO JAN FOR MX. UPON INVESTIGATION OF PROB, THE MX NOTED OIL ON THE OUTSIDE OF THE A/C AND INSIDE THE COWL. THE OIL FILTER BY-PASS INDICATOR WAS POPPED INDICATING FILTER BY-PASS. THE FILTER WAS REMOVED FOR INSPECTION AND SOAP SAMPLE, WHEN IT WAS REMOVED, IT WAS NOTED THAT IT HAD SUBSTANTIAL AMOUNTS OF METAL IMPREGNATED IN IT. THE APPROPRIATE ACTIONS ARE BEING TAKEN PRESENTLY.

PILOT BEGAN FEELING VIBRATION FROM LEFT ENGINE, LEFT GENERATOR HOT LIGHT CAME ON, FOLLOWED BY GENERATOR FAILURE.
NOSE WHEEL STEERING CIRCUIT BREAKER PN 727425, FAILED, CAUSING THE AIRCRAFT TO LOSE NOSE WHEEL STEERING CONTROL. AS A RESULT OF LOSING NOSE WHEEL STEERING, AIRCRAFT VEERED OFF TAXI WAY DURING TAXI FOR TAKEOFF.
