DURING INSPECTION, BOTH AFT EMERGENCY LIGHT LENSES WERE FOUND TO BE CRACKED. THE LENSES WERE REPLACED IN ACCORDANCE WITH THE MAINTENANCE MANUAL. NO FURTHER DEFECTS WERE NOTED.
DURING INSPECTION, THE EMERGENCY LIGHT POWER SUPPLY 11LB FAILED A FUNCTIONAL CHECK. THE POWER SUPPLY WAS REPLACED IN ACCORADANCE WITH THE SAAB AMM. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION, THE EMERGENCY LIGHTING FORWARD FLOOR STRIP DID NOT COME ON WHEN COCKPIT OR THE FLIGHT ATTENDANT SWITCH WAS SELECTED ON. THE FORWARD FLOOR PROXIMITY LIGHT STRIP WAS REPLACED. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION, THE EMERGENCY LIGHT LOWER LEFT HAND FAILED A FUNCTIONAL CHECK. FURTHER, THE EMERGENCY LIGHT POWER SUPPLY 15LN FAILED A FUNCTIONAL CHECK. THE POWER SUPPLY 15LN WAS REPLACED. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION, THE EMERGENCY LIGHT BATTERY PACK 11LN FAILED A FUNCTIONAL CHECK. THE BATTERY PACK WAS REPLACED IN ACCORDANCE WITH THE SAAB AMM.
DURING INSPECTION, THE EMERGENCY LIGHT BATTERY PACK 15LN FAILED A FUNCTIONAL CHECK. THE BATTERY PACK WAS REPLACED IN ACCORDANCE WITH THE SAAB AMM.
THE CREW NOTED THAT THEY HAD INCORRECT FLAP INDICATIONS WHILE CONFIGURING THE AIRCRAFT FOR LANDING. THE FLAPS INDICATED 13 WHEN SET TO 15 DEGREES AND 18 WHEN FLAPS 20 WAS SELECTED. THE CREW DECLARED AN EMERGENCY AND RETURNED TO FIELD. THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE PERFORMED OPERATIONAL CHECKS OF THE FLAP SYSTEM WITH NO DEFECTS NOTED.

DURING INSPECTION, CORROSION WAS FOUND ON THE ANGLE AT LT WING WS 337.0. THE ANGLE WAS REPLACED IN ACCORDANCE WITH THE SRM.
DURING INSPECTION, CORROSION WAS FOUND ON THE ANGLE AT LT WING WS 337.0-343.0 . THE ANGLE WAS REPLACED IN ACCORDANCE WITH THE SRM.
DURING INSPECTION, THE EMERGENCY LIGHT POWER SUPPLY 17LN FAILED A FUNCTIONAL CHECK. THE POWER SUPPLY WAS REPLACED. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION, THE CABIN EMERGENCY LIGHTS WERE FOUND TO BE INOPERATIVE WITH CIRCUIT BREAKER E9 POPPED. MAINTEANCE REPLACED THE 9LN EMERGENCY POWER SUPPLY. OPERATIONAL CHECK S WERE GOOD WITH NO FURTHER DEFECTS NOTED.
ON DESCENT, THE CREW NOTED THAT NP STARTED ROLLING BACK AT APPROXIMATELY 10,000 FEET. AT AROUND 3,000 FEET, THE RIGHT HAND ENGINE FAILED AND AUTO SHUTDOWN. THE AIRCRAFT IS CURRENTLY OUT OF SERVICE AND A CORRECTIVE ACTION WILL BE SUBMITTED AS A SUPPLEMENT TO THIS REPORT.
THE CREW REPORTED THAT ON DESCENT, A R ENG MTX MESSAGE APPEARED AND NP DECREASED. THE CREW FOLLOWED THE QRH AND RESET THE FADEC WITH NO CHANGE. THE ENGINE HAD A SELF SHUTDOWN SHORTLY AFTERWARD. THE ENGINE WAS RESTARTED PER THE QRH. THE R ENG MTX MESSAGE REMAINED ON FOR THE DURATION OF THE FLIGHT. THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE RECALIBRATED THE NUMBER 2 BETA TUBE AFTER CONSULTING WITH ROLLS ROYCE. AN OPERATIONAL CHECK FLIGHT WAS SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING INSPECTION, THE EMERGENCY LIGHT POWER SUPPLY, 9LN, FAILED A FUNCTIONAL CHECK OF THE SYSTEM. MAINTENANCE REPLACED POWER SUPPLY 9LN. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.

EMERGENCY LIGHTING POWER SUPPLY BATTERY 17LN FAILED FUNCTION TEST. REPLACED WITH SERVICEABLE UNIT.
R/H NACELLE ENGINE FIRE EXTINGUISHER BOTTLE SUPPORT BRACKET HAS CORROSION. REPLACED PER WORLDWIDE AIRCRAFT SERVICES ENGINEERING ORDER
THE CREW REPORTED THAT THE LEFT HAND INBOARD LANDING GEAR DOOR WAS OPEN ON LANDING. MAINTENANCE LUBED THE GEAR DOOR. THEN THEY PERFORMED EXTENSION AND RETRACTION OF THE LANDING GEAR TO VERIFY ADJUSTMENT IN ACCORDANCE WITH THE SAAB MAINTENANCE MANUAL. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION, THE EMERGENCY LIGHT POWER SUPPLY, 9LN, FAILED A FUNCTIONAL CHECK OF THE SYSTEM. MAINTENANCE REPLACED POWER SUPPLY 9LN. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION, THE CREW NOTED CIRCUIT BREAKER E9 WAS OUT. THE CIRCUIT BREAKER WAS RESET. DURING THE RESET THE FA PANEL EMERGENCY PUSH BUTTON LIGHT FLASHED AND THE CIRCUIT BREAKER POPPED AGAIN. MAINTENANCE PERFORMED AN OPERATIONAL TEST OF THE EMERGENCY LIGHT SYSTEM AND DETERMINED THAT RELAY 75LN HAD FAILED. THE RELAY WAS REPLACED. OPERATIONAL CHECKS WERE GOOD WITH NOT FURTHER DEFECTS NOTED.
DURING INSPECTION, THE PILOT EMERGENCY DOOR WAS FOUND TO BE LEAKING WATER. MAINTENANCE OPENED THE PILOT EMERGENCY DOOR, INSPECTED THE SEAL, AND RESECURED. A SPLASH TEST WAS SATISFACTORY, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING INSPECTION, THE EMERGENCY LIGHT POWER SUPPLY , 13LN, FAILED THE EMERGENCY LIGHT TEST. MAINTENANCE REPLACED THE POWER SUPPLY. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION, ISSUES WERE FOUND WITH THE STANDBY INSTRUMENT LIGHTING SYSTEM. MAINTENANCE REPLACED THE EMERGENCY LIGHTING POWER SUPPLY 11LN. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
THE CREW REPORTED THAT THEY RECEIVED TWO MAINTENANCE CAUTIONS. ONE FOR L ELEVATOR G AUG AND ANOTHER FOR L ELEVATOR IAS AUG. THE CREW DECLARED AN EMERGENCY AND RETURNED TO FIELD. MAINTENANCE REPLACED THE LEFT-HAND POWER ELEVATOR CONTROL UNIT. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
THE CREW REPORTED THAT THEY RECEIVED A RIGHT-ENGINE FIRE WARNING MESSAGE IN FLIGHT. THE CREW DECLARED AN EMERGENCY. THE AIRCRAFT LANDED AT ITS INTENDED DESTINATION WITHOUT FURTHER INCIDENT. MAINTENANCE REPORTED NO EVIDENCE OF FIRE FROM THE ENGINE. MAINTENANCE REPLACED THE FIRE DETECTOR CONTROLLER UNIT. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED. FURTHER, AN OPERATIONAL CHECK FLIGHT WAS PERFORMED WITH NO FURTHER ISSUES.
THE CREW REPORTED THAT THEY RECEIVED AN APU FIRE MESSAGE DURING FLIGHT AND DISCHARGED THE APU FIRE BOTTLE. THE AIRCRAFT IS CURRENTLY OUT OF SERVICE AND THE FINAL CORRECTIVE ACTION WILL BE SUBMITTED AS A SUPPLEMENT TO THIS REPORT.
DURING A FUNCTIONAL TEST OF THE EMERGENCY LIGHT BATTERY PACKS, PACKS 9LN, 11LN, 15LN AND 17LN FAILED. MAINTENANCE REPLACED THOSE 4 EMERGENCY LIGHT BATTERY PACKS IN ACCORDANCE WITH THE MAINTENANCE MANUAL. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
THE CREW REPORTED THAT DURING FLIGHT THE LEFT MAIN LANDING GEAR DOORS (BOTH DOORS) WERE NOT CLOSING AFTER GEAR EXTENSION. THE AIRCRAFT LANDED WITHOUT INCIDENT AT ITS INTENDED DESTINATION. MAINTENANCE CLEANED AND LUBED THE LEFT HAND MAIN LANDING GEAR FORWARD ACTUATING RODS. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION, THE EMERGENCY LIGHT CIRCUIT BREAKER NUMBER 9 WAS FOUND BE TRIPPED. MAINTENANCE REPLACED EMERGENCY LIGHT SWITCH ASSEMBLY. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED AT THAT TIME. BEFORE THE AIRCRAFT FLEW, MAINTENANCE FOUND THE CIRCUIT BREAKER TO BE TRIPPED AGAIN. MAINTENANCE REPLACED RELAY 75LN. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
THE CREW REPORTED THAT THEY RECEIVED A L PROP OIL PRESSURE LOW CAS CAUTION MESSAGE. THEY RETURNED TO FIELD AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE LEFT-HAND GEARBOX OIL ABSOLUTE PRESSURE SENSOR. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
THE CREW REPORTED THAT AFTER TAKE OFF THEY RECEIVED A GEAR FAULT CAUTION MESSAGE UPON RETRACTING THE LANDING GEAR. THEY RETURNED TO FIELD WITHOUT FURTHER INCIDENT. MAINTENANCE FOUND A STICKING LEFT-HAND UPLOCK SWITCH. THE SWITCH WAS CLEANED AND LANDING GEAR SWINGS PERFORMED WITH NO FURTHER DEFECTS NOTED.
THE CREW REPORTED THAT AFTER TAKE OFF THE CO PILOT ND SCREEN WENT BLANK. THE CREW RETURNED TO FIELD AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE RIGHT-HAND MFD. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
THE CREW REPORTED THAT AFTER TAKE-OFF, ALL INSTRUMENT LIGHTS FLICKERED. A LEFT-HAND GENERATOR FAULT MESSAGE APPEARED ALONG WITH SEVERAL OTHER ASSOCIATED MESSAGES. THE CREW RETURNED TO FIELD LANDED WITHOUT FURTHER INCIDENT. THE LEFT HAND GENERATOR WAS REPLACED. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED. `
DURING INSPECTION THE FORWARD PROXIMITY LIGHTING STRIP WAS FOUND TO BE PULLED AWAY FROM THE MOUNT AND THE WIRES WERE PULLED OUT. MAINTENANCE REPLACED THE FORWARD PROXIMITY LIGHT STRIP. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
DURING INSPECTION, THE MECHANIC DISCOVERED THE FORWARD CABIN FLOOR PROXIMITY LIGHT 31LN IS INOPERATIVE AND DETACHED FROM THE SIDE OF THE AISLE. MAINTENANCE SOLDERED DETACHED WIRES ON THE FLOOR PROXIMITY LIGHT STRIP. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED.
"DURING INSPECTION, THE MECHANIC FOUND THE FLIGHT ATTENDANT EMERGENCY LIGHT SWITCH 7LN ""ARMED"" PORTION OF SWITCH IS INOPERATIVE, AND BULB IS BURNED OUT. FINAL CORRECTIVE ACTION AND PART INFORMATION WILL BE SUBMITTED AS A SUPPLEMENT TO THIS REPORT.
DURING FLIGHT, THE CREW RECEIVED A CABIN AUTO PRESSURIZATION FAIL MESSAGE. THE CREW FOLLOWED THE QUICK REFERENCE HANDBOOK AND THE MESSAGE DID NOT CLEAR. THEY DIVERTED THE AIRCRAFT AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE REPLACED THE AUTO PRESSURIZATION CONTROLLER. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED ***THE FINAL CORRECTIVE ACTION AND PART INFORMATION IS BEING SUBMITTED AS A SUPPLEMENT TOT HE REPORT ORIGINALLY FILED ON 7/18/2022.***
DURING INSPECTION, THE EMERGENCY LIGHTS WERE FOUND TO BE INOPERATIVE AT ROWS THREE AND FOUR. MAINTENANCE REPLACED THE NUMBER 2 EMERGENCY LIGHT BATTERY. OPERATIONAL CHECKS FAILED. MAINTENANCE THEN REPLACED THE NUMBER 3 INVERTER. OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED. ****THE FINAL CORRECTIVE ACTION IS BEING SUBMITTED AS A SUPPLEMENT TO THE ORIGINAL REPORT FILED ON 12/17/21. PART INFORMATION HAS ALSO BEEN UPDATED.
DURING INSPECTION, SEVERAL EMERGENCY LIGHTS WERE FOUND TO BE INTERMITTENTLY INOPERATIVE IN THE FRONT SECTION OF THE AIRCRAFT. A LOOSE CONNECTOR WAS RESECURED AND OPERATIONAL CHECKS WERE GOOD WITH NO FURTHER DEFECTS NOTED. *NOTE* THE FINAL CORRECTIVE ACTION IS BEING SUBMITTED AS A SUPPLEMENT TO THE ORIGINAL REPORT FILED ON 11/21/2021.
THE CREW REPORTED UNRELIABLE AIRSPEEDS AND ALTIMETER AT FL370, UNTIL ABOUT 12,000 FEET. THE CREW ALSO REPORTED AN IC 1 WOW INOP MESSAGE AND THAT THEIR CAPTAIN'S PRIMARY FLIGHT DISPLAY HAD RED X'S. THE CREW DECLARED AND EMERGENCY AND DIVERTED TO KSGF, LANDING WITHOUT INCIDENT. THE CREW FURTHER REPORTED AN ELECTRICAL SMOKE SMELL IN THE COCKPIT ON TAXI IN. THE AIRCRAFT IS CURRENTLY OUT OF SERVICE.
CREW REPORTED LEFT WINDSHIELD HEAT WAS INOPERATIVE AND FOGGING UP. MAINTENANCE FOUND WINDSHIELD HEAT FUSE 100FU01-080 FAILED (OPEN), THIS WAS CAUSED BY THE OVERHEATING OF HEAT CONTROL CONTACTOR 480CC01. THE CONTACTOR TERMINAL âAâ HAD OVERHEATED CAUSING MELTING OF WIRE INSULATION AND DAMAGING THE TERMINAL (SEE PHOTO). THE CREW DID NOT REPORT ANY BURNING OR UNUSUAL ODORS IN THE COCKPIT. THE CONTACTOR AND FUSE WERE REPLACED AND SYSTEM OPERATIONAL CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE.
DURING CRUISE, BAGGAGE SMOKE CASS MESSAGE, MASTER WARNING, BAG SMOKE LIGHT TURNED ON, LATER EXTINGUISHED. AIRCRAFT LANDED AT KAUS WITHOUT INCIDENT. BAGGAGE COMPARTMENT WAS INSPECTED WITH NO EVIDENCED OF FIRE OR SMOKE. MAINTENANCE TROUBLESHOOTING PROBLEM, WILL REPORT FINDINGS WHEN AVAILABLE. MAINTENANCE COULD NOT DUPLICATE THE REPORTED PROBLEM. REMOVED AND REPLACED THE FORWARD SMOKE DETECTOR IN ACCORDANCE WITH THE EMB-145 AMM 26-15-01 AS PRECAUTIONARY MAINTENANCE. SYSTEM OPERATIONAL CHECKED GOOD.
FLIGHT KAUS - KTUSâ BAGGAGE SMOKEâ MASTER WARNING MESSAGE ILLUMINATED ON EICAS WHILE IN CRUISE AT FL 280. CREW FOLLOWED QRC AND DISCHARGED BAGGAGE COMPARTMENT FIRE EXTINGUISHER BOTTLES. DECLARED AN EMERGENCY AND DIVERTED TO KELP AND LANDED WITHOUT INCIDENT. FIRE DEPARTMENT INSPECTED BAGGAGE COMPARTMENT AND FOUND NO EVIDENCE OF FIRE OR SMOKE. MAINTENANCE FOLLOWED THE FIM REFERENCE: 26-15-00-810-805-A. THIS LEAD TO A TRANSITORY CONDITION OF THE BAGGAGE SMOKE DETECTION SYSTEM ELECTRICAL CIRCUIT (WM 26-15-50). CLEANED BOTH SMOKE DETECTORS AND PERFORMED OPERATIONAL CHECK OF SYSTEM IAW AMM 26-15-00-700-804-A. SYSTEM CHECKED GOOD. BAGGAGE COMPARTMENT FIRE EXTINGUISHER BOTTLES WERE REPLACED AND AIRCRAFT RETURNED TO SERVICE.
WHILE ON SHORT FINAL TO KAUS, THE FLIGHT DECK CREW AND FORWARD FLIGHT ATTENDANT REPORTED AN ELECTRICAL BURNING ODOR. AIRCRAFT CONTINUED APPROACH AND LANDED WITHOUT INCIDENT. MAINTENANCE PERFORMED CHECKS IAW FIM 21-20-00 AND 25-31-00 TASK AND INSPECTED GALLEY AND UNDER FLOOR AREA. UNABLE TO LOCATE ANY SIGNS OR A SOURCE FOR ELECTRICAL OVERHEATING ODOR. AIRCRAFT RETURNED TO SERVICE.
AFTER TAKEOFF, CREW RECEIVED A LAVATORY SMOKE CDS MESSAGE. AIRCRAFT RETURNED TO DEPARTURE AND LANDED WITHOUT INCIDENT. MECHANIC ON BOARD FLIGHT REPORTED NO SMOKE IN LAVATORY OR ANY UNUSUAL ORDERS. MAINTENANCE TROUBLESHOT AND FOUND THE LAVATORY SMOKE PANEL AT FAULT. REMOVED AND REPLACED THE PANEL. AIRCRAFT RETURNED TO SERVICE.
AFTER TAKEOFF, CREW RECEIVED A NR 2 PACK 2 OVERLOAD AMBER CAS MESSAGE. FLIGHT ATTENDANT HEARD A LOUD BANG AND SMELLED ELECTRICAL SMOKE. CREW RETURNED TO DEPARTURE AND LANDED WITHOUT INCIDENT. MAINTENANCE REPORTED THE RIGHT AIR CYCLE MACHINE HAD FAILED. THE ACM WAS REPLACED.
FLIGHT ATTENDANT REPORTED THAT SMOKE WAS IN THE PASSENGER CABIN, AS THE AIRCRAFT WAS PASSING THROUGH 6000 FT. DECLARED AN EMERGENCY AND RETURNED TO DEPARTURE, LANDED WITHOUT INCIDENT. MAINTENANCE PERFORMED FIM 21-20-00 AND FOUND THE RIGHT AIR CONDITIONING PACK TO BE FAULTY. REMOVED AND REPLACES THE RIGHT AIR CYCLE MACHINE. AIRCRAFT RETURNED TO SERVICE.
