N48PL HAD AN RTO ON 12-21-2023. WHEN I GOT TO THE PLANE THE FLAPS WERE AT FLAPS 20, FLAPS COULD GO BACK TO THE FULL UP POSITION, BUT THEY WOULD NOT GO PAST 20 DEGREES, I WAS ABLE TO SEE THE FLAP FAIL CAS MESSAGE, I INTERROGATED THE CMC, I FOUND A BIT TEST FAILED ON THE FSECU, ONCE THE FSECU SENSES THE FLAPS FAILED, THE FLAPS WILL STAY ON THAT SPECIFIC CONFIGURATION, UNTIL THE POWER IS CYCLED. I PERFORMED A GENERAL VISUAL INSPECTION OF THE FLAPS FOR ANY ABNORMAL MECHANICAL FAILURE, NO FAULT FOUND. I SHUT DOWN THE APU & TURNED ELECTRICAL POWER OFF FOR 10 MINUTES, HAD THEY CREW PERFORM A HIGH-SPEED TAXI TO RECREATE THE CONDITIONS OF THE FAILURE, EVERYTHING WORKED. PLANE FLEW TO KSAN YESTERDAY AFTERNOON WITHOUT ANY FURTHER ISSUES.
PLANE DEPARTED FROM KHDN, AFTER TAKE OFF CREW NOTICED GEAR RED UNSAFE LIGHT, CREW CONTACTED TOWER TO VERIFY GEAR WAS UP, TOWER CONFIRMED THE LEFT GEAR WAS DOWN, CREW LOWERED GEAR DID A LOW APPROACH TO KHDN TO CONFIRM ALL 3 GEAR WERE DOWN & DECIDED TO DIVERT TO KLAS. PLANE LANDED WITHOUT ANY ISSUES.
CREW WAS FLYING AT FL370, FROM KCRQ TO MMHZ, APPROXIMATELY 40 NM FROM MMMZ THE COCKPIT OVERHEAD SPEAKERS STARTED MAKING THE SOUND OF INTENSE STATIC AT FULL VOLUME, CREW SWITCHED FROM COMM2 TO COMM 1 & CONFIRMED BOTH COMM 1 & COMM 2 WERE MAKING THE SAME STATIC SOUND, THE CABIN ATTENDANT NOTICED A ELECTRICAL SMELL IN THE GALLEY, PIC CONFIRMED THE SMELL, ORDERED THE CREW TO WEAR THEIR O2 MASK & DECLARED AN EMERGENCY THEN BEGAN THE EMERGENCY DESCENT INTO MMMZ. AIRCRAFT LANDED WITHOUT INCIDENT.
DURING THE TAKE OFF PHASE FROM APF TO CRQ, THE CREW NOTICED A "DOOR UNLOCKED" ANNUNCIATOR, THE CREW ABORTED THE TAKE AND TAXIED BACK TO THE FBO. CREW TESTED DOOR LOCKING MECHANISM AND WAS NOT ABLE TO DUPLICATE THE "DOOR UNLOCKED" ANNUNCIATOR, WHILE DOING ONE MORE WALKAROUND THE CREW NOTICED THE LEFT SIDE TIRES WERE DEFLATED. A FEW INTERNAL PARTS ON THE MED LATCHING MECHANISM WERE REPLACED BY JET EAST AS A PREVENTIVE ACTION.
ON APPROACH TO KAUS AT 5,000 FEET THE 'BAGGAGE SMOKE' LIGHT ILLUMINATED. POWER WAS SET AT ABOUT 85% TO MAINTAIN ALTITUDE AND ABOUT 210KTS. AT THIS POINT THE CREW DECLARED AN EMERGENCY & REDUCED POWER TO START DESCENDING SLOWING FOR APPROACH. AIRPLANE WAS LOOKED OVER, PARKED NORMALLY. THERE WAS NO EVIDENCE OF A FIRE/SMOKE & NO ODOR WAS PRESENT UPON LANDING.
PLANE WAS RELOCATING FROM HND TO CRQ, DURING TAKE OFF PHASE CREW NOTICED THE FUEL FILTER BYPASS LIGHT CAME ON THE RIGHT ENGINE. CREW ABORTED THE TAKE OFF & TAXI BACK TO FBO.
ON POST TAXI AT KLAX, CREW NOTICED DADC DATA INVALID FAULT MESSAGE, CREW NOTICED THE SAT & TAT WERE 98 DEGREES CELSIUS. AFTER 30 MIN SAT & TAT TEMPERATURES CAME BACK TO NORMAL. CREW INADVERTENTLY LEFT ADS HEATER PANEL SWITCH ARMED AFTER LANDING FOR AN EXTENDED PERIOD INDUCING THE SAT & TAT TEMPERATURE INVALID DATA.
AIRCRAFT TOOK OFF FROM KCRQ TO KSLC, AFTER APPROXIMATELY HALF AN HOUR INTO THE FLIGHT AT FL390 & CRUISE, CREW NOTICED L/E OIL PRESSURE INDICATOR CONSISTENTLY FLUCTUATES BETWEEN RED & GREEN, BUT NO ANNUNCIATOR MASTER CAUTION FLASHED. CREW FOLLOWED QRH & WAS ABLE TO CONTINUE WITHOUT ANY FURTHER ISSUES.
AT LEVEL FLIGHT CREW NOTICED THE RIGHT ENGINE OIL LOW PRESSURE ILLUMINATED, FOLLOWED BY A LOSS OF N1 AND SUBSEQUENT AMBER LIGHTS INDICATED FOR GEN FAILURE AND HYDRAULIC LOW. CREW DECLARED AN EMERGENCY AS HAD LOST POWER FROM THE RIGHT ENGINE, THEN STARTED A DECENT AND WENT THROUGH THE APPROPRIATE CHECKLISTS. R/E STILL HAD N1 & N2 ROTATION, CREW WAS ABLE TO RESTART THE ENGINE, THE ENGINE THEN INDICATED NORMAL ACROSS ALL GAUGES. THEREFORE THE CREW CONTINUED ONTO ITS DESTINATION OF KCRQ. AFTER CESSNA PERFORMED A FADEC DOWNLOAD & SENT IT TO WILLIAMS, WILLIAMS RECOMMENDED TO REPLACE THE FUEL DELIVERY UNIT.
BEFORE TAKE-OFF AT JAC, CREW PERFORMED BEFORE TAKE OFF CHECK LIST. RT TT2/PT2 SENSOR HEATER FAIL LIGHT CAME ON & STAYED ILLUMINATED. CREW RETURNED TO FBO.
15 MINUTES PRIOR TO LANDING, CREW NOTICED AN UNCOMMANDED PARTIAL SPEED BRAKES DEPLOYMENT. CREW PROCEEDED WITH CHECK LIST, COULD NOT GET SPEED BRAKES STOWED. LANDED SAFELY WITHOUT ANY ISSUES. AFTER LANDING, SPEED BRAKES WERE STILL PARTIALLY DEPLOYED.
AIRCRAFT WAS TAXIING (TRIP FROM KCRQ TO KSDL), CREW NOTICED THAT RT ENGINE STATER GENERATOR FAILED VOLT/AMP. CREW RESET GENERATOR ISSUE STAYED. CREW DECIDED TO RETURN TO FBO AT KCRQ.
AFTER A NORMAL TAKEOFF FROM KCRQ HEADED TO KLAS, CREW NOTICED A LEFT SIDE âFUEL LOW LEVELâ CAUTION LIGHT ILLUMINATED AFTER CLIMBING. THE CREW JUST HAD FUEL IN THE PLANE BEFORE TAKEOFF AND THE FUEL GAUGES READ NORMAL. CREW FOLLOWED THE CHECK LIST PROCEDURES FOR âFUEL LOW LEVELâ BUT THE CAUTION LIGHT REMAINED ILLUMINATED. CREW INFORMED ATC THAT AIRCRAFT NEEDED TO RETURN AS A PRECAUTION.
AFTER A SUCCESSFUL DEICE, OF 35 GALLONS, CREW TAXIED AT CENTENNIAL. AFTER TAKING OFF, CREW NOTICED THE RED LIGHT IN THE HANDLE WAS STILL ILLUMINATED. CREW PROCEEDED WITH CHECK LIST AND RECYCLED THE GEAR TWO TIMES WITH THE SAME RESULT, RED LIGHT IN THE GEAR HANDLE. AT THAT POINT, CREW DECIDED TO RETURN TO CENTENNIAL FIELD, AIRCRAFT LANDED WITH ANY TROUBLE. WEST STAR APA JACKED THE AIRCRAFT & CYCLED THE GEAR MULTIPLE TIMES (13) WITHOUT ANY FAULTS. .... .. . NOTE: WE SUSPECT ICING WAS A FACTOR
CREW WAS RE-POSITIONING N233MW FROM KSBA TO KCRQ. DURING THE TAKE-OFF PHASE, THE CREW NOTICED A FLASHING MASTER CAUTION WARNING AND THE R EFC OFF ANNUNCIATIOR ILLUMINATED. CREW DECIDED TO ABORT THE TAKE-OFF. AFTER EXITING THE RUNWAY, CREW PROCEEDED TO RE-TEST THE EFC CHECK & THIS TIME THE TEST PASSED & THE CREW COMPLETED THE RE-POSITIONING FLIGHT WAS WITHOUT ANY PROBLEMS.
WHILE DESCENDING TO PSC, "BRAKE FAILED" CAS MESSAGE APPEARED. PILOT INFORMED TOWER ABOUT ISSUE & ASKED TO GO AROUND. WHILE GOING AROUND PILOT FOLLOWED CHECKLIST FOR "BRAKE FAILED", THEN A "LOW HYDRAULIC LOW PRESSURE" CAS MESSAGE APPEARED, PILOT FOLLOWED CHECKLIST FOR THE SECOND CAS MESSAGE. PILOT REQUESTED EMERGENCY VEHICLE & TOW TUG ON STANDBY THE RUNWAY. UPON LANDING PILOT USED EMERGENCY BRAKE TO STOP THE AIRCRAFT, AFTER STOPPING THE AIRCRAFT SAFELY. THE AIRCRAFT GOT TOWED AWAY FROM THE RUNWAY.
AIRCRAFT TOOK OFF FROM KCRQ TO KFAT, ON CLIMB CREW NOTICED "HYD PRESS" LIGHT ON, THEY PERFORMED CHECKLIST PROCEDURES TO RESET, THEY WERE UNABLE TO RESET. THEY RETURNED TO KCRQ.
FROM KAUS TO KCRQ, DURING THE APPROACH, A "FLAP FAIL" CAS MESSAGE WAS DISPLAYED. THE APPROACH WAS CANCELLED, AFTER THE PILOTS FOLLOWED THE QRH PROCEDURES THE FAULT WENT AWAY. THE AIRCRAFT LANDING WITHOUT ANY PROBLEM.
DURING TAKEOFF FROM SJC TO CRQ, CREW NOTICED A "DOOR UNLOCKED" LIGHT ON. TAKE OFF WAS REJECTED. CREW TAXI BACK TO FBO. CREW RECHECKED ALL DOORS & WARNING LIGHT WENT OFF. AIRCRAFT TOOK OFF NORMALLY AFTER THAT.
ON A FLIGHT FROM KCRQ TO KJAC, WHILE PERFORMING ENGINE START "LT GEN FAIL" CAS MESSAGE APPEARED ON MFD. FLIGHT WAS CANCELLED.
ON THE FLIGHT FROM KLAS TO KVNY, ON SHORT FINAL "YAW DAMP FAIL" CAS MESSAGE CAME ON PFD.
DURING TAXI AT KBUR HEADED TO KEUG, YAW DAMPENER DISCONNECTED MULTIPLE TIMES DURING TAXI, CAS MESSAGE "YD DIS" APPEARED ON MFD. ABORTED TAKE-OFF, TAXIED BACK TO FBO.
AFTER AIRCRAFT TOOK OFF FROM KCRQ TO KSMX, PILOT RETRACTED GEAR, BUT GEAR INDICATION DISAGREED. PILOT DECIDED TO RETURN TO KCRQ.
FLIGHT GOING FROM CRQ TO SNA, AFTER TAKE OFF GEAR WOULD NOT COME UP, CAME BACK IMMEDIATELY TO CRQ.
AFTER TAKEOFF FROM NAPC TO NSNA, GEAR WOULD NOT GO UP. PILOT CALLED TOWER & WAS TOLD TO DO COME BACK TO NACP & DO AN UNSCHEDULED LANDING BACK TO NAPC.
BEFORE TAKE OFF CREW NOTICED A RT THRUST ATTENUATOR UN-STOWED LIGHT WAS ILLUMINATED.PLANE TAXIED BACK TO THE HANGAR.
DURING DESCENT WITH AUTOPILOT ENGAGED, PLANE OSCILLATES.
PLANE TOOK OFF FROM KTRK IN DIRECTION TO KYVR, WHILE IN FLIGHT RIGHT HAND THRUST ATTENUATOR DEPLOYED, THE FLIGHT WAS DIVERTED TO KRNO.
RIGHT FUSELAGE BLEED WARNING CAME ON.
LANDING GEAR WOULD NOT RETRACT. WOW INPUT DISAGREEMENT. REPLACED INTERNAL SEAL.
WOW MISCOMPARE, AMBER MESSAGE ON TAKEOFF, LANDING GEAR DID NOT RETRACT.

RIGHT 10TH STAGE DUCT FAIL LIGHT ILLUMINATED DURING FLIGHT. TROUBLESHOT AND FOUND NO BLEED AIR LEAKS FROM RT 10TH STAGE MANIFOLD. FALSE INDICATION. REPLACED ALL OVERHEAT DETECTOR LOOPS FOR RT 10TH STAGE BLEED AIR LEAK DETECTION SYSTEM.
UPON TAKE-OFF, ACFT ACCELERATION DETERMINED SLOW BY PILOT AND COPILOT. AT 80 KNOTS INDICATED, TAKEOFF ABORTED. CREW SLOWED AIRCRAFT TO STOP USING ACFT BRAKES. LT MAIN TIRE DEFLATED AT WALKING SPEED, FROM FUSE PLUG RELEASE, TIRE BEAD SEPARATED FROM WHEEL. ACFT UNABLE TO BE ROLLED BECAUSE BRAKES FUSED TOGETHER, ACFT REMOVED FROM RUNWAY USING DOLLIES. TROUBLESHOT BRAKE SYS, FOUND NO FAULT WITH PARKING BRAKE VALVE, PARKING BRAKE CONTROL CABLE, EMERGENCY BRAKING SYS, BRAKE CONTROL VALVE. IN PROCESS OF INSPECTING LT WHEEL AND REPLACING LT AND RT BRAKES.
