IDLER GEAR [PN: 896804-2] TEETH GALLED DUE TO BEARINGS (2EA) [PN: 3103035-1, SN: 10-151725-07307 & 10-151725-07323, TSN: 5,558.3] WORN AND SPALLING. RESULTED IN UNCOMMANDED SHUTDOWN OF ENGINE DUE TO FUEL PRESSURE LOSS.
ELEVATOR TRIM TAB CABLE BROKE AFTER TAKEOFF
APPROXIMATLY 100 MILES FROM THE DESTINATION OF THE FLIGHT, THE PILOT NOTICED A OIL PRESSURE LOW CAUTION LIGHT FOR THE LT ENGINE. THE PILOT SECURED THE ENGINE AND FEATHERED THE PROPELLER, ACFT LANDED AT DESTINATION WITHOUT INCIDENT. MAINTENANCE FOUND THE ENGINE OIL LEVEL LOW. OIL WAS SERVICED TO THE REQUIRED LEVEL. ENGINE WAS GROUND RAN AND LEAK CHECKED. NO LEAKS FOUND AND ALL ENGINE CONDITIONS WERE NORMAL. ACFT WAS RETURNED TO SERVICE.
ENROUTE, EXPERIENCED A LIGHTING STRIKE APPROXIMATELY 70-80 MILES FROM DESTINATION. THIS STRIKE KNOCKED OUT BOTH COMMS. PLACED CORRECT CODE ON THE TRANSPONDER AND CONTINUED ON AND MADE AN UNEVENTFUL LANDING. MAINTENANCE DISCOVERED THAT THE NR 1 COMM ONE COAX, AND BOTH COMM ANTENNAS HAD BEEN DISABLED BY THE LIGHTING STRIKE.
DURING APPROACH, TURNED ON THE ENGINE INTAKE HEAT. RT ENGINE THEN INGESTED ICE AND FLAMED OUT. PILOT WENT AROUND, RESTARTED ENGINE AND LANDED WITHOUT FURTHER INCIDENT.
CUSTOMER REPORTED, LT CREW DOOR BLEW OPEN DURING FLIGHT CAUSING EXTENSIVE DAMAGE TO THE OUTER WINDOW, DOOR FRAME AND OUTER SKIN. DOOR WAS SENT TO US FOR REPAIR. AFTER DISASSEMBLY OF DOOR, FOUND BOLT, PN: AN3H HOLDING FRONT BELL CRANK, WHICH OPERATES THE FRONT 2 LATCHES, BROKEN IN HALF JUST BELOW THE SHANK. WITH THIS BOLT BROKEN THE FRONT LATCHES WILL NOT ENGAGE/DISENGAGE, THUS NOT ALLOWING DOOR TO BE SECURED IN THE ACFT WHEN FULLY CLOSED AND LOCKED. DURING FULL PRESSURIZATION THE DOOR COULD THEN BLOW OPEN. IT IS RECOMMENDED THAT OWNERS THAT HAVE A LT OR RT CREW DOOR (EMERGENCY EXIT) INSTALLED, PLACE DOOR IN CLOSED/LOCKED POSITION AND VISUALLY VERIFY THAT ALL LATCHES ARE FULLY ENGAGED BEFORE EACH FLIGHT.
DURING CRUISE, CREW DOOR BLEW OPEN CAUSING THE ACFT TO DEPRESSURIZE. DECLARED AN EMERGENCY, DESCENDED, AND RETURNED TO POINT OF TAKEOFF. A SAFE LANDING WAS MADE. ON INVESTIGATION DOOR WAS FOUND TO IN THE LATCHED-LOCKED POSITION. AS OF YET NO REASON FOR FAILURE HAS BEEN DETERMINED.
AT CRUISE FLIGHT, AFTER DEPARTING ENROUTE, THE COPILOTS OUTER WINDSHIELD SHATTERED AT FL 22500. ACFT RETURNED TO DEPARTURE WITHOUT INCIDENT. R & R THE COPILOTS WINDSHIELD IAW THE MM CHAPTER 56-10-00.
DURING INTIAL CLIMB CABIN DOOR WAS MAKING A WHISTLING SOUND THAT DEVELOPED INTO A HOWL. CABIN WOULD NOT MAINTAIN MORE THAN A 4.0 PSI DIFFERENTIAL. RETURNED TO BASE. INSPECTED SEAL AND FOUND IT MISALIGNED AT TOP MIDDLE POSITION, ADJUSTED SEAL AND APPLIED PRC TO TOP OF DOOR FRAME, IAW MM 52-10-50.
DURING INITIAL CLIMB, THE CREW NOTICED THE RT ENGINE OIL PRESSURE GUAGE HAD DROPPED TO ZERO PSI AND IT WAS NOT ASSOCIATED WITH A RT OIL PRESS ANNUNCIATOR. 20 SECONDS LATER THE CREW NOTICED A HARD BY-PASS AT ROUGHLY 630 DEGREES EGT. CREW INFORMED TOWER AND RETURNED BACK TO DEPARTURE WITHOUT INCIDENT. R & R THE RT ENGINE OIL PRESSURE TRANSDUCER IAW THE MM 79-30-01. GROUND RUN AND OPS CHECKS GOOD.
WHILE ENROUTE, THE CREW NOTICED AN ELECTRICAL BURNING ODOR IN THE COCKPIT. A PRECATIONARY LANDING WAS MADE TO INVESTIGATE. MX INSPECTED BEHIND ALL CIRCUT BREAKER PANELS,UNDER THE FLOOR,AND IN BOTH BATTERY BOXES WITH NO INDICATION OF OVERHEATING OR BURNING OF ANY KIND. THE ACFT WAS THEN GROUND RAN AND A FUNTION CHECK OF ALL SYSTEMS WAS COMPLETED WITH NO ABNORMAL INDICATIONS NOTED. AS OF THIS TIME THE PROBLEM CAN NOT BE DUPLICATED AND NO SOURCE HAS BEEN FOUND. ACFT RETURNED TO SERVICE.
PILOT SELECTED GEAR DOWN AND NEVER GOT A GREEN DOWN AND LOCKED INDICATION FOR THE NOSE GEAR. ALL OTHER INDICATIONS SUGGESTED THAT GEAR WAS DOWN AND LOCKED. ACFT MADE AN UNEVENTFUL LANDING. TROUBLESHOOTING BY MX FOUND WIRE ON LANDING GEAR ACTUATOR WAS STARTING TO BREAK CAUSING INTERMITTENT POWER TO THE DOWN LIGHT. A NEW SWITCH WAS INSTALLED, AND ALL GEAR CHECKS WERE SAT.
ACFT ON FINAL APPROACH, WHEN PILOT HEARD A THUD. STATED HE THOUGHT HE HAD HIT A BIRD. UNEVENTFUL LANDING WAS MADE. PILOT DEPARTED AC AND FOUND LT ENGINE TOP COWLING HAD COME OPEN. MX FOUND OUT, BAD LATCHING MECHANISM BROKE. SUSPECT LATCH BROKE AND THE COWLING BLEW OPEN ENOUGH FOR THE OTHER LATCH TO OPEN, ALLOWING TOP COWL TO BLOW BACK IN AIRSTREAM. THE BACK HINGE AND ALL LATCHES WERE REPLACED WITH SERVICEABLE PARTS. (K)
ENGINE STARTED RUNNING ROUGH AND PILOT NOTICED OIL LEAKING ON COWLING. ENGINE WAS SECURED AND A UNEVENTFUL LANDING MADE. MAINT FOUND CYL NR 4 HAVING BROKEN STUDS AND CYLINDER STARTING TO SEPARATE FROM ENGINE. ALSO FOUND ROCKER BOX COVER SCREWS LOOSE/MISSING. ENGINE REMOVED FOR REPAIR. (K)
ON DESCENT TO DESTINATION, ENGINE BYPASS LIGHT ILLUMINATED, ATTEMPTED TO PULL POWER BACK, BUT ENGINE SEEMED TO NOT RESPOND AND TEMP STARTED TO RISE. ENGINE SECURED AND UNEVENTFUL LANDING MADE. ON GROUND, NOTICED OIL LEAKING FROM ENGINE. FOUND ENGINE WOULD NOT ACTIVATE. ENGINE REMOVED FOR FURTHER INSPECTION AND LOANER INSTALLED. (K)
WHILE TRAINING FOR A CHECK RIDE, POWER WAS PULLED BACK TO SIMULATE ENGINE AT CONDITION. WHEN POWER WAS REAPPLIED, ENGINE FAILED TO RESPOND ENGINE WAS SECURED, TROUBLESHOOTING DECIDED PROP GOV FAILED. PROP GOV WAS REPALCED WITH OVERHAULED UNIT, ALL CHECK SAT. SUSPECT BROKEN COUNTERWEIGHT IN GOVN WAS THE PROBLEM. (K)
BRAKE FIRE AFTER TAKEOFF. DISCOVERED AFTER ARRIVAL, BRAKE WAS SATURATED WITH BURNED HYDRAULIC FLUID, BOTH LOWER BRAKE LINES DAMAGED AND NR 4 TIRE IS BURNED TO CORD. (K)
RT ENGINE FINE WARNING FLICKERED ONE TIME IN FLIGHT. PILOT SECURED ENGINE AND FEATHERED PROP. UNEVENTFUL LANDING MADE AT MAINTENANCE INSPECTED ENGINE AND FOUND NO EVIDENCE OF FIRE OR FIRE DAMAGE. GROUND RUN ENGINE COULD NOT DUPLICATE PROBLEM. (K)
APPROXIMATELY 15 MINUTES OUT, THE CHIP LIGHT ILLUMINATED AND OIL PSI STARTED TO DROP. ENGINE SECURED AND PROP FEATHERED. AIRCRAFT LANDED. PULLED CHIP DETECTER AND OIL FILTER AND FOUND METAL IN BOTH. APPEARS A BEARING IN THE GEAR BOX FAILED. ENGINE TO BE REMOVED. (GL11200412580)
PILOT STARTED TO ENCOUNTER ICE AND TURNED ON THE ENGINE ANTI-ICE. BOTH ENGINES FLAMED OUT DUE TO CLEAR ICE THAT WAS UNKNOWN TO PILOT. PILOT RESTARTED BOTH ENGINES AND MADE A PRECAUTIONARY LANDING. GROUND CHEEKS WERE MADE OF THE DEICE SYSTEM, ENGINE ANIT ICE SYSTEM, AND IGNITION SYSTEM. ALL CHECKED SATISFACTORY. AIRCRAFT RETURNED TO SERVICE.
AFTER LEVELING OFF, ENGINE STARTED TO RUN ROUGH AND BACKFIRE. SECURED ENGINE AND FEATHERED PROPELLER. MAINTENANCE FOUND CYLINDER NR 1 CRACKED. REPLACED CYLINDER WITH AN OVERHAULED UNIT. AIRCRAFT GROUND RUN AND CHECKED SATISFACTORY.
WHILE EN ROUTE, ENGINE STARTED TO RUN ROUGH. ENGINE WAS SECURED. FOUND NR 11 CYLINDER HEAD CRACKED. REPLACED NR 11 CYLINDER WITH A YELLOW TAGGED UNIT. ENGINE OPS CHECKED SATISFACTORY.
NOSE GEAR LIGHT WOULD NOT COME ON, CYCLED GEAR 3 TIMES THEN NOSE LIGHT CAME ON. REMOVED AND REPLACED RT DOWN POSITION SWITCH, CYCLED GEAR SEVERAL TIMES AND OPS CKS GOOD.
RT ENGINE TORQUE STICKS AT 20 PERCENT. REMOVED AND REPLACED RT TORQUE GAUGE, OPS CKS GOOD.
ENGINE LOST FUEL FLOW IN CRUISE FLIGHT. ENGINE SHUT-DOWN AND FEATHERED. TROUBLESHOOTING DETERMINED FUEL CONTROL FAILED. THE FUEL CONTROL UNIT HAD APPROXIMATELY 450 HOURS SINCE REPAIR BY WOODWARD.
DURING GROUND CHECKS, FLAPS GIVE FULL TRAVEL. IN-FLIGHT, FLAPS WILL NOT OPERATE ABOVE AIRSPEEDS OF 147 KNOTS WITH GEAR UP. DURING LANDING IN PHX, FLAPS WORKED WITH GEAR DOWN AT 160 KTS. TROUBLESHOT SYSTEM. TESTED LEFT FLAP LOCK OUT VALUE. TESTED GOOD. REMOVED AND REPLACED FLAP CONTROL VALVE. RAN FLAPS AND GEAR SEVERAL TIMES UP AND DOWN. OPS CKS GOOD ON GROUND. NO LEAKS NOTED AT FLAP CONTROL VALVE OR FLAP LOCKOUT VALUE.
AFTER TAKEOFF, RIGHT ENGINE STARTED TO BACKFIRE AND RUN ROUGH. ENGINE WAS SHUT DOWN AS A PRECAUTIONARY METHOD. UPON TROUBLESHOOTING, FOUND LT MAG BAD. MAG WAS REPLACED WITH A YELLOW TAGGED UNIT. ENGINE WAS GROUND RUN, CHECKED SATISFACTORY. AIRCRAFT WAS RETURNED TO SERVICE.
LEFT ENGINE DOES NOT MAKE POWER. PERFORMED COMPLETE GROUND RUN ON LEFT AND RIGHT ENGINES. LEFT ENGINE MADE TARGET TORQUE (90 PERCENT AT 19 DEG OAT/500 PA) WITH P/L TRAVEL AND POWER TO SPARE. SEE FL 09643.
