THIS A REPEAT ITEM DATED 12/04/2025 - UNIQE CONTROL # S9BA202500003. CHECKED & INSPECTED FUEL NOZZLE SYSTEM AND FOUND FUEL NOZZLES NR 3, 4, 10, & 11 LEAKING BLEED, R & R FUEL NOZZLE GASKETS ) ON NOZZLES #3, 4, 10, & 11 IAW WC # 73-01-01 OP CK GOOD AT THIS TIME.
FLT/CREW REPORTED - UPON LEVELING OUT ON FLIGHT NR BB-136 SSP / SSB RT ENGINE FIRE WARNING LIGHT CAME ON WITH AURAL FIRE BELL WARNING CAME ON ALSO. POWER LEVER / TQ WAS REDUCE AND BOTH FIRE WARNING EXTINGUSHED, F/O VISUALLY CONFIRMED NO FIRE OR SMOKE WAS COMING OUT OF RT ENGINE. ALL ENGINE INSTRUMENTS PARREMETERS NORMAL AND IN GREEN ARC AREA, WITH NO OTHER WARNINGS WERE NOTICED AT THAT TIME. CONTINUE FLIGHT TO ITS FLIGHT DESTINATION.
03/07/2025 CREW REPORTED:'AILERON ACT. TRIM CB POPPED IN FLIGHT ELECTRICAL BURNING SMELL." MAINTENANCE INSPECTED DEFERRED THIS ITEM AS PER MEL 27-2. MAINTENANCE CORRECTIVE ACTION:" INSPECTED AILERON TRIM SYSTEM FOUND CONNECTION AT SWITCH LOOSE REPOSITIONED AND SECURED CONNECTION IAW DHC-6-300 M/M CHAPTER 27-10. THIS ACTION CLEAR THE MEL 27-2 DATE 03/08/2025.
CORROSION BEYOND MFR LIMITS ON STRINGERS AND INTERCOSTALS BETWEEN FS262 AND FS288 BETWEEN WL -4.68 AND -10.28. FOLLOWING PART NUMBERS WERE REMOVED AND REPLACED WITH NEW IAW DHC SRM. C6FS1620-29, C6FS1620-39, C6FS1620-31, C6FS2448-39, C6FS1620-37.
CORROSION FOUND BEYOND MFR LIMITS ON LH WING WS310.O FORE FLAP HINGE SKIN DOUBLERS. FOLLOWING PART NUMBERS WERE REMOVED AND REPLACED IAW DHC SRM. 969401-7 AND 969401-9.
CREW REPORTED; WHEN RIGHT FUEL LEVER SELECTED OFF, RIGHT ENGINE DID NOT SHUT DOWN. MAINTENANCE INSPECTED ENGINE CONTROL CABLES AND FOUND CABLES IN BIND. FURTHER INSPECTION FOUND THE RIGHT OUTBOARD ENGINE MOUNT BROKEN. REMOVED AND REPLACED ENGINE MOUNT IAW AMM 71-20, OPS CHECK GOOD.
IN SHALLOW CLIMB A DUPLICATABLE VIBRATION CAN BE FELT IN THE YOKE AND AIRPLANE. INSPECTION OF THE AIRCRAFT FOUND THE LH ELEVATOR TRIM TAB ATTACH BRACKET BROKEN. REMOVED AND REPLACED ELEVATOR TRIM TAB ASSEMBLY IAW DHC-6-300 AMM CH 27-30. OPERATIONAL CHECK AND FUNCTIONAL TEST FLIGHT GOOD.

WITH THE GEAR HANDLE UP THE NOSE GEAR DOWNLOCK LIGHT REMAIN ILLUMINATED.
UPON LANDING AND WHEN AIRCRAFT NOSE WHEEL TOUCHDOWN A VIBRATION AND LOUD NOISE WAS PRODUCE AND FELT FROM THE SAME AREA
DURING FLIGHT HYDRAULIC CAUTION LIGHT ON WITH HYDRAULIC PUMP ACTUATION WITH LOSS OF ALL HYDRAULIC ACCUMULATOR PRESSURE. MAINTENANCE PERSONNEL FOUND BLOWN HOSE ASSY AT THE NOSE RETRACT ACTUATOR.
EMERGENCY LIGHT ABOVE 2A SEAT INOP.
DURING HEAVY CHECK INSPECTION FOUND O/B RIB L/H AILERON WITH CORROSION BEYOND LIMITS
LAVATORY SMOKE LIGHT COMING ON FOR ABOUT 30 SECONDS AND THEN INTERMITTENT. CORRECTIVE ACTION: MX PERSONNEL INSPECTED LAVATORY SMOKE DETECTOR AND TESTED OK. NO DEFECTS FOUND.
DURING APPROACH, AFTER GEAR DOWN AND LOCKED "HYD" WARNING CAME ON. HYD. QTY. AND PRESSURE READ 0. MAINTENANCE PERSONNEL FOUND BLOWN HOSE ASSY. AT THE NOSE GEAR RETRACT ACTUATOR.
DURING FINAL DESCENT, FLIGHT ATTENDANT REPORTED SMELL OR ODOR COMING FROM 9 TO 11. CA: AFTER DVI FOUND FLUORESCENT TUBE LAMP HOLDER AT SEAT 9A BURNT. REMOVED AND REPLACED LAMP HOLDER.
DURING HMBV INSPECTION FOUND LEADING EDGE ASSY.(RT) CORROSION OUT OF LIMITS. ASSEMBLY WAS REMOVED AND REPLACED.
UNSAFE GEAR INDICATION ON RT MAIN LANDING GEAR.
NOSE GEAR INDICATOR LIGHT FACE PLATE CAME OFF REPLACED BULBS.
WHEN TAXIING TO THE GATE AND BEFORE ENGINE SHUTDOWN, THE PILOTS EXPERIENCED A STRONG ELECTRICAL ODOR IN THE COCKPIT. FOUND THE PROCESSING UNIT.
WINDOW LIGHTS AT ROWS 6 AND 7 SMELL LIKE BURNING PLASTIC.
EMERGENCY EXIT LIGHT OVER ROW 6A INOPERATIVE. FOUND THE CONNECTOR UNSECURE.
DURING TAKE OFF, THE PROPELLER DID NOT FOLLOW CONDITION LEVER POSITION. THE LINK AT THE PROPELLER CONTROL UNIT WAS FOUND SEPARATED DUE TO A LOCK WASHER THAT FAILED.
ON CLIMB GUST LOCK LIGHT CAME ON, AFTER REACHING CRUISE THE CREW BECAME AWARE THAT THERE WAS NO RESPONSE TO THE RUDDER INPUT.
DURING HEAVY CHECK INSPECTION, FOUND CORROSION AND A PUNCTURE ON THE FUSELAGE SKIN LT SIDE BETWEEN BS 316 - 319. ACCOMPLISHED REPAIR IAW 340SDS042295.
PASSENGERS REPORT AN ODOR OF SMOKE FROM ROWS 1 TO 5. FOUND SEIZED LEFT AIR CYCLE MACHINE. THE ACM WAS REMOVED AND REPLACED.
ON TAKEOFF, RIGHT T HANDLE FIRE WARNING.
DURING FLT, RT BLEED AIR LEAK LIGHT ILLUMINATED. CREW COMPLETED QRH 1-6, LIGHT EXTINGUISHED AND FLIGHT CONTINUED. ACFT WAS RETURNED TO HOME BASE USING A FERRY PERMIT. DURING PRELIMINARY INSPECTION FOUND RT BLEED AIR RUBBER DUCT BROKEN. DUCT WAS REPLACED. OPS AND LEAK CHECK PERFORMED FOUND WORKING PROPERLY. ALSO ACFT WAS RELEASED FOR FUNCTIONAL CHECK FLIGHT AND NO ABNORMALITIES FOUND DURING THIS FLIGHT.
BLEED AIR LEAK CAUTION LIGHT ILLUMINATED DURING FLIGHT, PROCEED TO QRH AND LIGHT REMAINED ILLUMINATED, PROCEEDED WITH ENGINE SHUTDOWN. PRELIMINARY INSPECTION FOUND SENSING ELEMENT CHAFING AGAINST DUCT CLAMP. SENSING ELEMENT REPOSITION AND SECURED, OPS CHECK PERFORMED AND FOUND SYSTEM WORKING PROPERLY.
RIGHT HOT PIPE LIGHT CAME ON AFTER TAKEOFF 2,000 FT. R & R WARNING ELECTRONIC COMPUTER.
(CGKBR) LEFT ENGINE NOT MAKING 50 PERCENT TORQUE ON LAST FLIGHT CHECKS. REPLACED FUEL PUMP AND FUEL CONTROL.
LT ENGINE ONLY PRODUCING 40 LBS TORQUE ON TAKEOFF. TAKEOFF WAS ABORTED. TORQUE TRANSDUCER WAS REPLACED IAW AMM 79-31.
WHEN ENROUTE, CAPTAIN WINDSHIELD CRACKED. DECLARED EMERGENCY AND LANDED. THE WINDSHIELD WAS REPLACED.
DENT ON SEAM AFT OF 9TH WINDOW RT SIDE. EVALUATED DAMAGE AT FUSELAGE BS 516. REPAIRED IAW SRM 53-70-10-205, FIG 203. NDT INSPECTED IAW NDT MANUAL 51-06-00. PRIMED AND PAINTED TO MATCH ACFT COLORS. NO CHANGE IN ACFT WEIGHT AND BALANCE.
DENT AT RT FUSELAGE UNDER 11TH CABIN WINDOW. EVALUATED DAMAGE AT FUSELAGE BS 580.0, WATERLINE 100. REPAIRED IAW SRM 53-70-10-205, FIG 203. NDT MANUAL 51-06-00. PRIMED AND PAINTED TO MATCH ACFT COLORS. NO CHANGE IN ACFT WEIGHT AND BALANCE.
AIRCRAFT WILL NOT TURN LEFT ON GROUND USING NOSE WHEEL STEERING. R & R STEERING SYSTEM CONTROL MICRO SWITCH AND PERFORMED OPS CHECK GOOD IAW MM 32-50-35-02.
LEFT ENGINE STAGE 1 ROTOR BLADE MISSING MATERIAL. DAMAGE DISCOVERED VIA BORESCOPE INSPECTION. PICTURE TAKEN AND FORWARDED TO TECH SUPPORT AND THEY DETERMINE THAT ENGINE NEED TO BE REPLACE.
STRANGE AND VERY LOUD GRINDING COMING FROM NOSE WHEELS AREA DURING TAXI INTO THE RAMP. INSPECTED AND FOUND NR 1 NOSE WHEEL BEARING BROKEN. R & R NR 1 NOSE WHEEL ASSY IAW AMM 32-41-15-04. OPS CHECK GOOD.
LEFT NACELLE OTBD LONGERON CRACKED. REMOVED CRACKED LONGERON, LOCATED NEW LONGERON AND INSTALLED. REF AC 43.13-1B
LT ENGINE LOST POWER ON TAKEOFF, AUDIBLE GRINDING NOISE AND EXCESSIVE VIBRATION CAN BE HEARD AND FELT COMING FROM ENGINE.
DURING MAINTENANCE, FOUND THAT F/A JUMPSEAT DOES NOT LOCK IN POSITION. AFTER INSPECTION AND TROUBLESHOOTING FOUND STRIP-CHAFING AND PACKER BROKEN NOT ALLOWING TO SLIDE THE SEAT TO THE SIDE AND ALSO WAS UNABLE TO KEEP SEAT ON POSITION, TOP PORTION OF THE SEAT GOES FORWARD.
