ON JUNE 12, 2026, DURING DESCENT ON FLIGHT MTN8380 FROM MEM TO DHN, 714FX, EXPERIENCED INTERMITTENT FUEL LEVEL LO R AND FUEL LEVEL LO L AMBER CAS MESSAGES BEGINNING APPROX 15 MINS PRIOR TO LANDING WITH 3,210 LBS INDICATED FOB AT DEPARTURE. THE RIGHT TANK LEVEL-LOW INDICATION APPEARED FIRST AT APPROX 900 LBS INDICATED, FOLLOWED APPROX FIVE MINS LATER BY THE LT TANK AT 950 LBS INDICATED RT AND 880 LBS LT. THE LEFT-SIDE MESSAGE CYCLED ON AND OFF DURING BANKED TURNS TO INTERCEPT THE ILS FOR RUNWAY 32 AT DHN. CREW EXECUTED THE APPLICABLE CHECKLIST. AIRCRAFT LANDED NORMALLY. INDICATED FOB ON ARRIVAL WAS 1,830 LBS WITH THE LEFT-SIDE CAS MESSAGE STILL PRESENT.
DURING CLIMB OUT, EXPERIENCED LOSS OF OIL PRESSURE FROM #2 ENGINE, SHUT DOWN #2 ENGINE AND RETURNED TO FIELD WITHOUT FURTHER EVENT. FROM CREW: ON THE LEG FROM CHS TO MEM CLIMBING THROUGH 16,000 FEET FOR 20,000 FEET, CREW FELT A SUDDEN YAW AND WERE ALERTED BY A CAP MESSAGE ALERTING ELEC. CREW'S ATTENTION THEN WENT TO THE NP GAUGE WHICH READ AROUND 20%. TORQUE GAUGE WAS BROUGHT TO FDAU BUG AFTER RISING DUE TO NP DECREASING. CREW PERFORMED APPROPRIATE CHECKLISTS FOR THE ISSUE AND BEGAN TO TURN BACK TO CHS. ENGINE OIL DROPPED BELOW 40PSI AND TRIGGERED LOW ENGINE OIL LIGHT. AT THIS TIME, CREW PERFORMED ENGINE SHUTDOWN CHECKLIST AS WELL AS SINGLE ENGINE PROCEDURES CHECKLIST AS WELL AS DECLARED EMERGENCY. ALL OTHER CHECKLISTS WERE COMPLETED BEFORE LANDING AND TRUCKS WERE ROLLED BEHIND OUT OF PRECAUTION. CREW TAXIED SINGLE ENGINE TO FEDEX RAMP WHERE ENGINE WAS INSPECTED BEFORE SHUTDOWN. MX FOUND BROKEN O-RING ON #2 PCU, R+R PCU, OPS CHECK GOOD
WHEN N743FX LANDED IN MCO LAST NIGHT WITH THE LOW FUEL LIGHT ON, WHEN THE PIC WAS PULLING INTO PARK, THEY HAD APPROXIMATELY 700 LBS AND THE NR 2 ENGINE SHUT OFF. THE RT SIDE NR 4 PROBE SEEMS TO BE WAY OFF PER 28-40-00.
DURING MTNA'S 800 HR INSPECTION ON 758FX IT WAS IDENTIFIED THAT: LEFT FUEL QTY FOUND TO BE INACURATE, WITH EMPTY LEFT SIDE IS 77OLB OF FUEL MX: SYSTEM T/S PERFORMED, DETERMINED #4 PROBE AT FAULT PER C408 MM CHAPT 28-40-00. REPLACED #4 LEFT FUEL QTY PROBE AND SYSTEM CHECKED GOOD PER C408 MM CHAPT 28-40-05 AND 28-40-00.
NOXIOUS VAPOR IN CABIN. FLAP CIRCUIT BREAKER POPPED. THE PILOT NOTED AN UNKNOWN AIRBORNE IRRITANT IN THE COCKPIT WHICH RESULTED IN A BURNING SENSATION IN THE EYES, NOSE AND THROAT. THE PIC DECLARED AN EMERGENCY AND RETURNED TO EWR AND LANDED SAFELY. MX: 1) PERFORMED ENGINE COMPARTMENT INSPECTION IAW C208 AMM 71-00-01. NO DEFECT NOTED. PERFORMED ENGINE RUN IAW MANUFACTURE. PILOTS OPERATING HANDBOOK. NO NOXIOUS FUMES OR OTHER SMELLS DETECTED. LUBRICATED FLAP BELL CRANK IAW C208-MM-12-21-02. FLAP OPERATED FREE AND CORRECT. PERFORMED OPS CHECK ON FLAP NORMAL AND STBY FLAP SYSTEM, CYCLED BOTH NORMAL AND STBY SYSTEM INDIVIDUALLY, IAW MM-27-50-00, ALL FLAP POSITION CYCLED WITHOUT POPPING FLAP CB.
NORMAL BRAKING UNRESPONSIVE ON BOTH CAPT SIDE AND F/O SIDE. REPLACEMENT OF THE HYDRAULIC METERING VALVE (6027GG) IAW TASK ATR-A-32-42-50-02001-520A-A/720A-A. LEAK CHECK GOOD AND OPS CHECK GOOD
ALTERNATOR FAILURE AFTER START. STANDBY POWER ON CONTINUOUSLY. REMOVED AND REPLACED ATANDBY ALTERNATOR AND BELT.
IN-FLIGHT ENGINE SHUTDOWN, FLIGHT 8850, TLH-MEM. ON THE VISUAL APPROACH TO RUNWAY 27 AT KMEM, THE CREW OBSERVED THE NO. 1 ENGINE TORQUE INDICATION BECOME ERRATIC, THEN DROP TO ZERO, ACCOMPANIED BY AN UNCOMMANDED LEFT ROLL. MEMORY ITEMS WERE ACCOMPLISHED, AND THE CREW SHUT DOWN THE NO. 1 ENGINE AND FEATHERED THE PROPELLER. AN EMERGENCY WAS DECLARED WITH ATC. THE CREW CONTINUED FOR THE LANDING AND COMPLETED A SINGLE-ENGINE LANDING ON RUNWAY 27 WITHOUT FURTHER INCIDENT. ONCE CLEAR OF THE RUNWAY, THE ENGINE WAS SECURED USING THE QRC/QRH. AIRPORT RESCUE/FIRE PERSONNEL REPORTED NO FIRE OR SMOKE OBSERVED, AND THE AIRCRAFT TAXIED IN AND PARKED.

DURING COMPLETION OF NDT TASK ATR-A-53-57-XX-03001-350A-A, FOUND SUBSURFACE CRACK IN HOLE #8 AS IDENTIFIED IN FIGURE ICN-ATR-A-535700-A-FB429-06PSI-A-001-1 (SHEET 1), THIS IS AT STATION 12791/FR27 ON THE LH SIDE CRACK EXTENDS AFT FROM THE CENTER OF THE HOLE APPROXIMATELY .5 INCHES AS IDENTIFIED BY THE EDDY CURRENT INSPECTION THE LEFT FRAME 27, BOLT AT HOLE 8 IS TEMPORARILY REINSTALLED UNTIL WE HAVE A RESOLUTION.

DURING CLIMB OUT FOR FLIGHT 8300 EWR-IAD, THE PIC IDENTIFIED THE TRANSPONDER WAS INOP AND COMPLETED AN AIR RETURN, DECLARING NO EMERGENCY AND LANDING UNEVENTFULLY.
DURING STARTUP OF AIRCRAFT N708FE ON FLIGHT 8129 BQN-POS, THE CREW NOTICED A HAZE IN THE COCKPIT THAT SMELLED LIKE SMOKE ALONG WITH THE CAPTAIN'S SIDE INTERCOM AND MICROPHONE NOT WORKING. THE CREW SHUTDOWN THE AIRCRAFT AND MAINTENANCE BEGAN TROUBLESHOOTING. BQN MAINTENANCE COMPLETED AN ENGINE RUN SUCCESSFULLY ON N708FE WITH NO SMOKE DETECTED.
SHORTLY AFTER LINE CHECK, CREW O2 PRESSURE WAS AGAIN BELOW 1500PSI AFTER BEING SERVICED JUST PRIOR. DURING INSPECTION FOUND OXGEN BOTTLE VALVE DAMAGED AND LEAKING. BOTTLE REQUIRES REPLACE. REMOVED AND REPLACED CREW OXYGEN BOTTLE (5101HT) W/ REF TO ATR-A-35-11-10-01001-520A-A/720A-A. PERFORMED LEAK CHECK, WITH NO DEFECTS NOTED.
LH MLG TIRE WAS BLOWN ON LANDING.
DURING THE APPROACH TO LAND, LDG GEAR HANDEL FAILED FOLLOWED BY 3 UNLOCK LIGHTS ON BOTH SYSTEMS; LDG GEAR GRAVITY EXTENSION HAS BEEN USED FOR LANDING. PROBLEM WAS FOUND TO BE A FAILURE OF THE LANDING GEAR SELECTOR VALVE. A SERVICABLE VALVE WAS INSTALLED AND AIRCRAFT WAS RETURED TO SERVICE.
SLOW CREW OXYGEN LEAK ON AIRCRAFT AT CYLINDER.
DURING INSPECTION OF ICE VANE ACTUATOR, FOUND THE ENGINE TRUSS ATTACH POINT IS CRACKED. PER SRM NO WELDED REPAIRS ALLOWED, WILL HAVE TO R & R ENGINE TRUSS.
ODOR OF SMOKE FOLLOWED BY RECIRC FAN #2 FAULTED IN FLIGHT.
DURING CLIMBOUT OF FLIGHT 7501 EYW-FLL, THE DOOR WARNING ANNUNCIATOR CAME ON. THE AIRCRAFT RETURNED BACK TO THE DEPARTURE AIRPORT AND INSPECTED THE DOOR HANDLE. ONCE THE DOOR HANDLE WAS INSPECTED AND THE ANNUNCIATOR EXTINGUISHED, THE AIRCRAFT WAS INFO-ED ONLY AND DEPARTED ONCE AGAIN.
DURING DECENT OF FLIGHT 7553 DLH-MSP WITH AIRCRAFT N914FX, THE CAPTAIN REPORTED THE NUMBER TWO OIL PRESSURE GAUGE WAS FLUCTUATING BETWEEN 0-20 PSI AND LOW OIL PRESSURE WAS INDICATED ON THE CAP. THE FLIGHT CREW PERFORMED THE #2 ENGINE SHUT DOWN PROCEDURES, AN EMERGENCY WAS DECLARED, AND AIRCRAFT 914FX LANDED UNEVENTFULLY IN MSP. MSP MAINTENANCE HAS IDENTIFIED A CRACKED OIL COOLER AS THE ISSUE AND PARTS HAVE BEEN ORDERED WITH THE EXPECTATION OF THE AIRCRAFT RETURNING TO SERVICE.
CRACKS HAVE BEEN FOUND ON THE SKIN BETWEEN THE VERTICAL STABILIZER ACCESS PANELS 347KL AND 347LL.
THE LH NLG WHEEL/TIRE ASSEMBLY SEPARATED FROM THE AIRCRAFT ON TAKEOFF AND LANDED ON AIRPORT PROPERTY.
ON FLIGHT 7326 BUF-IND WITH AIRCRAFT 821FX, THE FLIGHT CREW WAS UNABLE TO RETRACT THE MAIN LANDING GEAR ON CLIMB OUT AND DUE TO THIS, COMPLETED AN AIR RETURN TO BUF, DECLARING NO EMERGENCY AND LANDING UNEVENTFULLY. AIRCRAFT IS TO BE FERRIED TO HEAVY MAINTENANCE FACILLITY FOR FURTHER TROUBLESHOOTING.
DURING CLIMB OF FLIGHT 7129 POS-BQN, AIRCRAFT 800FX INCURRED A PITCH DISCONNECT MASTER WARNING ALERT. THE CREW PERFORMED THE QRH PROCEDURES, DECLARED AN EMERGENCY, AND RETURNED TO POS UNEVENTFULLY. CONTRACT MAINTENANCE WAS NOTIFIED AND TROUBLESHOT THE ISSUE. NO FAULTS WERE FOUND, AND AIRCRAFT WAS OPS CHECKED GOOD. FLIGHT 7129 DEPARTED A SECOND TIME BUT THE SAME ISSUE OCCURRED. THE CREW RETURNED TO POS WITH NO FURTHER ISSUES.

HOLE HAS BEEN OVERSIZED BY CUSTOMER TO 11.049MM (0.435â) TO REMOVE ALL DAMAGE. NO OFFSET VALUE HAS BEEN REPORTED. FOR ADDITIONAL DETAILS AND PICTURES, REFER TO SDR_00244960_01_A.
DURING CRUISE OF FLIGHT, THE PICâS FRONT WINDSHIELD PANE SHATTERED. DUE TO THE CONDITION OF THE WINDSHIELD AND A SLIGHT BURNING SMELL IN THE COCKPIT, THE CREW DESCENDED TO 10,000 FEET, DECLARED AN EMERGENCY, AND DIVERTED TO PTP. CORRECTIVE ACTION IN WORK.

DURING CRUISE OF FLIGHT 8106 BQN-BGI, AIRCRAFT 909FX INCURRED A HIGH ITT ON THE RIGHT ENGINE WHICH RESULTED IN A DECLARED EMERGENCY AND DIVERSION INTO STX. AIRCRAFT 909FX LANDED UNEVENTFULLY. REMOVED AND REPLACED THE #2 ENGINE FUEL NOZZLE KIT.
PERFORMED INSPECTION OF THE ICE VANE ACTUATOR MOUNT STRUCTURE FOR CRACKING. CRACK WAS FOUND ON THE #2 ICE VANE ACTUATOR STRUCTURE. PERMANENT REPAIR TO BE COMPLETED. PERFORMED MARKING OF THE CRACK ENDS AS INSTRUCTED IN THE ENGINEERING LETTER. REINSPECTION TO BE PERFORMED EVERY 25 FLIGHT HOURS FOR CRACK GROWTH OR ADDITIONAL DAMAGE.
UPON POST FLIGHT INSPECTION IN CHS, THE FO DISCOVERED THAT THE LEFT HAND ENGINE COWLING ON THE NUMBER 2 ENGINE WAS MISSING. BOTH PRE FLIGHT INSPECTIONS BY BOTH CREW MEMBERS CONFIRMED THAT THE COWLING WAS ATTACHED PRIOR TO FLIGHT. THE COWLING APPEARS TO HAVE BEEN RIPPED OFF IN FLIGHT WITH SECONDARY DAMAGE SUSPECTED ON THE TOP OF THE ENGINE COWLING AND POSSIBLY THE LEADING EDGE OF THE WING. WE SUSPECT THE DAMAGE WAS CAUSED BY SEVERE TURBULENCE DURING THE INITIAL CLIMB. UPON DISCOVERY OF THE MISSING COWLING, THE PIC WAS NOTIFIED, WHO THEN IMMEDIATELY INFORMED OPERATIONS AND MAINTENANCE CONTROL.
ICE VANE ACTUATOR MOUNT AND STRUCTURES ON #2 ENGINE INSPECTION PERFORMED. FOUND CRACKS UNDER THE RH ENGINE ACTUATOR MOUNT. PERFORMED MARKING OF CRACK ENDS AS INSTRUCTED IN TEXTRON ENGINEERING LETTER REF DQ43241M1 DATED 2/27/2024. REINSPECTION TO BE PERFORMED EVERY 25 FLIGHT HOURS.
PERFORMED INSPECTION OF THE ICE VANE ACTUATOR MOUNT STRUCTURE FOR CRACKING. CRACK WAS FOUND ON THE #2 NACELLE UNDER THE RIGHT HAND SIDE OF THE ICE VANE ACTUATOR MOUNT. REMOVED THE #2 AFT LOWER COWLING AND WAS SENT FOR REPAIR. RECEIVED SERVICEABLE COWLING AND INSTALLED. PERFORMED GROUND RUNS. NO LEAKS NOTED AT THIS TIME. SUCCESSFUL OPS CHECK COMPLETED.
ON TAXI OUT, SMOKE WAS OBSERVED IN THE COCKPIT. THE #2 AUTO TEMP SELECT FAULTED WITH ELECTRIC SMOKE ON CAP. CORRECTIVE ACTION IN WORK.
AFTER TAKEOFF, LEAVING MAX RPM, RT ENGINE BEGAN SURGING. THE FLIGHT CREW DECLARED AN EMERGENCY AND RETURNED TO GSP UNEVENTFULLY. CLEANED CANNON PLUG CONNECTIONS ON RT ENGINE ECU. PERFORMED ENGINE RUN. UNABLE TO REPLICATE ENGINE SURGING. NO FURTHER DEFECTS NOTED.
ICE VANE ACTUATOR MOUNT AND STRUCTURES ON #1 ENGINE INSPECTION PERFORMED. FOUND CRACKS ON THE INBOARD ACTUATOR MOUNTING STRUCTURE LOWER SIDE. ASSISTED TEXTRON GO TEAM IN REPAIRS BY REMOVING AFT LOWER COWLING. AFT LOWER ENGINE COWLING REPAIRED UNDER TEXTRON WORK ORDER 203447, REPAIR STATION NUMBER CNQ918C. RECEIVED SERVICEABLE AFT LOWER COWLING AND REINSTALLED IAW CESSNA 408 MM 71-10-15. PERFORMED GROUND ENGINE RUNS, OPS CHECKS GOOD.
ICE VANE ACTUATOR MOUNT AND STRUCTURES ON #2 ENGINE INSPECTION PERFORMED. FOUND CRACKS ON THE IBOARD ACTUATOR MOUNTING PLATE LOWER SIDE. FOUND CRACK ON #1 ENGINE NACELLE AS WELL, REMOVED BOTH #1 AND #2 AFT LOWER COWLINGS AND GAVE TO TEXTRON GO TEAM FOR REPAIRS, SEE TEXTRON WORK ORDER 203405 REPAIR STATION NUMBER CNQR918C, RECEIVED SERVICABLE AFT LOWER COWLINGS FROM GO TEAM AND INSTALLED BOTH #1 AND #2 AFT LOWER ENGINE COWLINGS. RAN ENGINES AT GROUND IDLE, FOUND LEAK FROM #1 ENGINE COOLER LINES. RETORQUED LINES AND RAN ENGINES, ALL OPS CHECKS GOOD.
DURING THE CLIMB TO CRUISE ALTITUDE, AT APPROXIMATELY FL190, THE CREW RECEIVED AN EXCESS CABIN ALTITUDE ALERT. AFTER PERFORMING THE ASSOCIATED QRH ITEMS, THE CREW NOTIFIED ATC, DESCENDED TO 10,000 FT, AND THE FLIGHT RETURNED TO THE FIELD WITHOUT FURTHER INCIDENT.
O2 BOTTLE AND REGULATOR LEAKING. REPLACED O2 BOTTLE AND O2 REGULATOR. OPS AND LEAK CHECK GOOD.
